分析HbA1c检测及胰岛素释放试验诊断2型糖尿病的价值

摘要 目的研究糖化血红蛋白(HbA1c)检测及胰岛素释放试验诊断2型糖尿病的价值。方法将2019年3月—2020年3月在该院就诊的100例2型糖尿病患者纳入观察组,按照空腹胰岛素分泌水平分为1组和2组,对照组为同期在该院进行健康体检的健康人群50名。将各组的检查结果进行比对。结果观察组1组、2组的HbA1c水平、空腹血糖、胰岛素抵抗指数(HOMA-IR)均高于对照组,胰岛β细胞功能指数(HOMA-β)低于对照组,观察组1组的空腹胰岛素>对照组>观察2组,观察组1组的HbA1c低于观察2组,HOMA-β高于观察2组,差异有统计学意义(P<0.05);观察1组各时间点的胰岛素水平均高于观察2组,观察1组空腹时、2 h、3 h的胰岛素水平高于对照组,30 min、1 h的胰岛素水平低于对照组,观察2组3 h的胰岛素水平高于对照组,其他时间点均低于对照组,差异有统计学意义(P<0.05);随着口服葡萄糖后时间的延长,血糖呈上升趋势,从2 h到达峰值后开始下降,丙组血糖水平高于乙组、甲组、对照组,差异有统计学意义(P<0.05)。结论HbA1c检测、胰岛素释放试验能够有效观察血糖、胰岛功能的变化,为2型糖尿病的诊治提供可靠的数据支持。 Objective To study the value of glycosylated hemoglobin(HbA1c)detection and insulin release test in the diagnosis of type 2 diabetes.Methods 100 patients with type 2 diabetes who were treated in the hospital from March 2019 to March 2020 were included in the observation group.They were divided into groups 1 and 2 according to their fasting insulin secretion levels.The control group was healthy in the hospital during the same period.Fifty healthy people underwent physical examination.Compared the inspection results of each group.Results The HbA1c level,fasting blood glucose,and insulin resistance index(HOMA-IR)of the observation group 1 and 2 were higher than those of the control group,and the isletβ-cell function index(HOMA-β)was lower than the control group.The fasting insulin of the observation group 1 group>the control group>the observation group 2;the HbA1c of group 1 was lower than that of observation group 2,and HOMA-βwas higher than that of observation group 2.The difference was statistically significant(P<0.05);the insulin level of observation group 1 was higher than that of observation group 2 at each time point,and the insulin levels of the observation group 1 of fasting,2 h,and 3 h were higher than those of the control group,and the insulin levels at 30 min and 1 h were lower than those of the control group.The 3 h insulin levels of the observation 2 groups were higher than those of the control group,and the other time points were lower than those of the control group.The difference was statistically significant(P<0.05).With the prolongation of the time after oral glucose,blood glucose showed an upward trend,and began to decrease after reaching the peak at 2 h.The blood glucose level of group C was higher than that of group B,group A,and control group,and the difference was statistically significant(P<0.05).Conclusion HbA1c detection and insulin release test can effectively observe the changes of blood glucose and islet function,and provide reliable data support for the diagnosis and treatment of type 2 diabetes.
