对糖尿病视网膜病变患者血糖管理中实施集束化护理的效果分析

Analysis of the Effect of Cluster Nursing in Blood Glucose Management of Patients with Diabetic Retinopathy

目的探讨糖尿病视网膜病变患者血糖管理中实施集束化护理的效果。方法选择2019年2月—2020年2月该院收治的86例糖尿病视网膜病变患者,随机分为集束化组(44例)和对照组(42例)。对照组按治疗需求常规护理,集束化组在对照组基础上增加集束化护理,对比两组血糖管理效果,同时对比两组患眼视力、自我管理能力、生存质量改善效果。结果干预3个月后,集束化组患眼视力(0.92±0.14)、自我管理行为评分(62.03±8.17)分和SQOLVI评分(168.25±14.02)分均显著高于对照组(0.71±0.17)、(51.02±9.57)分、(147.27±15.28)分,差异有统计学意义(P<0.05)。集束化组干预3个月后空腹血糖(4.96±0.58)mmol/L、餐后2 h血糖(7.72±1.13)mmol/L、糖化血红蛋白指标水平(5.93±1.13)%,显著低于对照组(5.89±0.97)、(8.92±1.35)mmol/L、(6.79±1.08)%,差异有统计学意义(P<0.05)。结论糖尿病视网膜病变患者临床护理中,根据患者个体情况,实施集束化护理,可有效提升患者血糖管理成效,并可促进患者改善,并提高自理管理能力和生存质量,有助于患者预后改善。

Objective To explore the effect of clustered care in blood glucose management of patients with diabetic retinopathy.Methods A total of 86 patients with diabetic retinopathy admitted to the hospital from February 2019 to February 2020 were selected and randomly divided into a cluster group(44 cases)and a control group(42 cases).The control group received routine care according to treatment needs,and the clustered group added clustered care based on the control group.The blood glucose management effects of the two groups were compared,and the visual acuity,the self-management ability and quality of life improvement effects of the two groups were compared.Results After 3 months of intervention,the visual acuity(0.92±0.14),self-management behavior score(62.03±8.17)points and SQOLVI score(168.25±14.02)points of the affected eyes in the clustering group were significantly higher than those of the control group(0.71±0.17),(51.02±9.57)points,(147.27±15.28)points,the difference was statistically significant(P<0.05).The fasting blood glucose(4.96±0.58)mmol/L,2 h postprandial blood glucose(7.72±1.13)mmol/L,and glycosylated hemoglobin index level(5.93±1.13)%in the cluster group after 3 months of intervention were significantly lower than those in the control group(5.89±0.97)mmol/L,(8.92±1.35)mmol/L,(6.79±1.08)%,the difference was statistically significant(P<0.05).Conclusion In the clinical care of patients with diabetic retinopathy,the implementation of clustered care according to the individual conditions of the patients can effectively improve the effectiveness of blood glucose management of the patients,promote the improvement of patients,and improve the ability of self-care management and the quality of life,which is helpful to improve the prognosis of patients.
