水疱性远端指炎1例及文献复习

A Case of Blistering Distal Dactylitis and Literature Review

患儿女,2岁,左手环指末端指节红肿、水疱伴疼痛1 d。皮肤科情况:左手环指末端指节弥漫性紫红色肿胀及水疱,触痛明显。皮肤镜检查显示:白色水肿背景上散在红色或紫红色出血,1个完整水疱含有黄白色脓液及壁内出血。血常规显示白细胞和中性粒细胞比例升高。疱液涂片见革兰阳性球菌,细菌培养有金黄色葡萄球菌生长。诊断:水疱性远端指炎。治疗:给予静脉滴注头孢唑林、外用呋喃西林及林可霉素凝胶,2周后皮损完全消退。

A 2-year-old girl presented with a one-day history of painful erythematous swelling with blistering on the left ring finger.Skin examination showed diffuse reddish/purple swelling on the distal volar aspect of the left ring finger,with obvious tenderness.Dermoscopy revealed sporadic reddish/purple hemorrhages and an intact blister containing yellowish-white pus with intramural purple hemorrhages on whitish edematous background.Complete blood count displayed leukocytosis with neutrophilia.Purulent smear displayed gram-positive cocci and bacterial culture grew Staphylococcus aureus.She was diagnosed as blistering distal dactylitis and treated with intravenous cefazolin,and topical furacilin solution and lincomycin gel.The lesions were completely resolved after two weeks.
