带量采购政策对我院降压药物利用状况的影响分析

Analysis of the Influence of Volume-based Drug Centralized Procurement Program on the Utilization of Anti-hypertension Drugs in a Tertiary Hospital

目的:分析带量采购政策对我院降压药物使用的影响,分析仿制药替代策略带来的经济效益,为完善相关药物政策提供理论依据。方法:对带量采购政策实施前8个月、第一批带量采购政策实施后8个月、第二批带量采购政策实施后8个月降压药物的销售金额、销售数量、用药频度(DDDs)、限定日费用(DDC)等进行动态分析,计算带量采购后的费用节省及费用节省率。结果:带量采购中选药品的价格显著降低,且续签后持续下降,单片药品价格降幅69%~92%;且未中选药品价格也有0~5%的下降。进入带量采购目录的降压药物DDDs下降幅度在32%~62%间,DDC明显降低,且续签后持续下降;未中选药品DDDs反而有所升高,DDC下降不明显。第一批带量采购政策实施后,目标药品实际节省费用为1869139.36元,费用节省率为16.17%;第二批带量采购政策实施后,目标药品实际节省费用为2654673.43元,费用节省率27.82%。结论:带量采购政策的实施对于降低高血压患者的治疗费用有显著意义,同时对医药市场药品价格存在一定冲击。

Objective:To analyze the impact of the volume-based drug centralized procurement program on the utilization of anti-hypertension drugs and economic effectiveness of generic drug substitution strategy,and to provide a reference for the improvement of medical related policy.Methods:Using pharmacoeconomics approach to have a dynamic analysis of the anti-hypertension drugs sales amount,consumption volumes,DDDs and DDC within 8 months before and after the first and second batches of centralized procurement starts.Calculating the cost saving and its rate after the implementation of drug centralized procurement program.Results:After the implementation of volume-based drug centralized procurement program,the price of the drugs in the procurement program dropped significantly,and continued to decrease after renewal,the price of single-piece drugs had fallen between 69%and 92%;And the prices of the drugs that were not selected also decreased by 0-5%.The DDDs of drugs in the procurement program generally showed a decline trend,ranging from 32%to 62%,DDC decreased significantly and continued to decline after renewal;while the DDDs of unselected drugs increased,the DDC also had no obvious drop.The actual cost saving of target drugs was RMB 1869139.36 after the first purchase with quantity,and the cost saving rate was 16.17%.The actual cost saving was RMB 2654673.43 after the second purchase with quantity,and the cost saving rate was 27.82%.Conclusion:The implementation of the volume-based drug centralized procurement program has significant benefits in reducing the treatment costs to hypertension patients,and at the same time it has a great impact on the drug pricing in the pharmaceutical market.
