对高龄产妇行无痛分娩联合助产士全程责任制的应用价值

The Application Value of Painless Childbirth Combined with Midwife’s Full Responsibility System for Elderly Parturients

目的:探讨对高龄产妇行无痛分娩联合助产士全程责任制的应用价值。方法:选取2018年10月―2020年7月于广州市第一人民医院产科收治的86例高龄产妇作为研究对象,按随机数表法分为对照组和观察组,每组各43例;对照组采用助产士全程责任制,观察组在对照组的基础上联合无痛分娩,对比两组各产程疼痛情况、产程所用时间及分娩结局。结果:观察组产妇各产程视觉模拟评分(VAS评分)均低于对照组(P<0.05);观察组产妇各产程所用时间均短于对照组(P<0.05);观察组阴道分娩率为93.02%,高于对照组的72.09%,差异有统计学意义(P<0.05);观察组胎儿宫内窘迫率为4.65%,低于对照组的20.93%,差异有统计学意义(P<0.05);观察组新生儿窒息率为2.33%,低于对照组的6.97%,但差异无统计学意义(P>0.05)。结论:对高龄产妇行无痛分娩联合助产士全程责任制有效缓解分娩疼痛,可提高阴道分娩率,降低胎儿宫内窘迫率。

Objective:To explore the application value of combined midwifery responsibility system in painless delivery for elderly women.Methods:A total of 86 elderly pregnant women admitted to the hospital from October 2018 to July 2020 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 43 cases in each group.The control group adopted the midwife responsibility system,and the observation group combined painless labor on the basis of the control group.The pain,labor time and labor outcome of the two groups were compared.Results:Visual analogue scale(VAS)scores of all stages in the observation group were lower than those in the control group(P<0.05).The duration of labor in the observation group was shorter than that in the control group(P<0.05).The rate of vaginal delivery in the observation group was 93.02%,higher than 72.09%in the control group,and the difference was statistically significant(P<0.05).The rate of fetal distress in the observation group was 4.65%,lower than 20.93%in the control group,and the difference was statistically significant(P<0.05).The rate of neonatal asphyxia in the observation group was 2.33%,lower than 6.97%in the control group,the difference was not statistically significant(P>0.05).Conclusion:The painless delivery combined with the midwife responsibility system for elderly women can effectively relieve labor pain,improve the rate of vaginal delivery and reduce the rate of fetal distress in utero.
