宫腔镜联合左炔诺孕酮宫内节育系统治疗子宫内膜息肉的临床价值分析

Value Analysis of Hysteroscopy Combined With Levonorgestrel Intrauterine Sustained-release System in the Treatment of Endometrial Polyps

目的探讨宫腔镜联合左炔诺孕酮宫内缓释系统治疗子宫内膜息肉的临床价值。方法选择2019年1月—2020年1月本院收治的重症子宫内膜息肉患者80例,按照随机数字表法分为两组,各40例。对照组实施单纯宫腔镜下息肉切除治疗,观察组联合术后宫腔内置入左炔诺孕酮宫内缓释系统,比较两组治疗3个月后不同观察时间点子宫内膜厚度及血红蛋白水平变化,比较两组随访期间出现的并发症,统计治疗后1年子宫息肉复发率。结果治疗1个月后、治疗3个月后,观察组子宫内膜厚度均显著小于对照组,差异有统计学意义(P<0.05),观察组血红蛋白水平均显著高于对照组,差异有统计学意义(P<0.05),观察组发生经期延长、反复贫血、盆腔感染及下腹痛的总比例显著低于对照组,差异有统计学意义(P<0.05),观察组治疗后1年子宫内膜息肉复发率显著低于对照组,差异有统计学意义(P<0.05)。结论对于子宫内膜息肉患者在实施宫腔镜术后联合应用左炔诺孕酮宫内缓释系统宫内置入,可调节子宫内膜厚底,提高机体血红蛋白水平,减少并发症及复发率。

Objective To explore the clinical value of hysteroscopy combined with levonorgestrel intrauterine sustained-release system in the treatment of endometrial polyps.Methods 80 cases with severe endometrial polyps admitted to our hospital from January 2019 to January 2020 were randomly divided into two groups,each with 40 cases.The control group was treated with hysteroscopic polypectomy alone,and the observation group was treated with levonorgestrel intrauterine slow-release system.The changes of endometrial thickness and hemoglobin level at different observation time points after 3 months of treatment were compared between the two groups.The complications occurred during follow-up were compared between the two groups,and the recurrence rate of uterine polyps was counted one year after treatment.Results After 1 month and 3 months of treatment,the endometrial thickness of the observation group was significantly lower than that of the control group(P<0.05),the hemoglobin level of the observation group was significantly higher than that of the control group(P<0.05),and the total proportion of prolonged menstruation,repeated anemia,pelvic infection and lower abdominal pain in the observation group was significantly lower than that in the control group(P<0.05).The recurrence rate of endometrial polyps in the observation group was significantly lower than that in the control group one year after treatment,and the difference was statistically significant(P<0.05).Conclusion For patients with endometrial polyps after hysteroscopy,combined application of levonorgestrel intrauterine slow-release system can adjust endometrial thickness,improve hemoglobin level,and reduce complications and recurrence rate.
