加速康复护理对食管癌患者术后胃肠功能及并发症的影响

Effect of Accelerated Rehabilitation Nursing on Postoperative Gastrointestinal Function and Complications of Patients With Esophageal Cancer

目的探究加速康复外科(enhanced recovery after surgery,ERAS)护理对食管癌患者术后胃肠功能及并发症的影响。方法纳入2019年1月—2020年12月本院收治的80例食管癌患者进行研究,均行胸腹腔镜联合手术治疗。按随机排列法分为对照组40例,予常规围术期护理;试验组40例,予ERAS护理。观察术后胃肠功能恢复情况、并发症(吻合口瘘、肺部感染、乳糜胸、误吸、腹胀、腹泻、尿路感染)发生情况以及术前(手术当天)、术后(术后3个月)生活质量(36-item short-form health survey,SF-36)评分变化。结果试验组肠鸣音恢复时间、排气时间、排便时间均短于对照组,差异显著(P<0.05);试验组术后并发症总发生率为10.00%,低于对照组27.50%,差异显著(P<0.05);术后,试验组SF-36评分高于术前,且高于对照组,差异显著(P<0.05)。结论ERAS护理能够促进食管癌患者术后胃肠功能的恢复,减少并发症的发生,利于其生活质量的提升。

Objective To study the influence of enhanced recovery after surgery(ERAS)on the postoperative gastrointestinal function and complications of esophagus cancer patients.Methods 80 esophagus cancer patients of undergoing the thoracoscopic and laparoscopic surgery from January 2019 to December 2020 in our hospital were investigated.According to random tabulation method,patients were assigned to control group(n=40,routine care)and test group(n=40,ERAS).The research results were concluded through the postoperative gastrointestinal function recovery conditions,incidences of complications like anastomotic fistula,pulmonary infection,chylothorax,attracts by mistake,abdominal distension,diarrhea,and urinary tract infections as well as SF-36 scores before surgery(upon the day of surgery)and after surgery(after 3 months of surgery).Results The bowel sound recovery time,exhaust time and defecation time in the test group were significantly shorter than control group(P<0.05);the overall complication rate in the test group(10.00%)was significantly lower than control group(27.50%)(P<0.05);SF-36 scores in the test group were significantly higher than before and control group(P<0.05).Conclusion ERAS can promote the gastrointestinal function recovery,reduce the surgical complications and improve the life quality for esophagus cancer patients.
