直肠癌患者在携带预防性回肠造口期间症状体验的质性研究

Symptoms experienced by parents with rectal cancer while carrying a prophylactic ileostomy:a qualitative study

目的:探讨直肠癌患者在携带预防性回肠造口期间的症状体验。方法:通过质性研究中的现象学研究方法,利用编制的半结构化访谈提纲,对17名直肠癌术后携带预防性回肠造口的患者进行深度访谈,并利用Colaizzi法进行资料整理分析。结果:通过分析得出4个主题,分别为复杂症状并存(伴随肛门和回肠造口特殊症状、造口排泄物量大和睡眠障碍交织)、消极自我感受(自尊受损、自责愧疚)、衍生出适应行为(寻求支持信息、改变膳食结构)、引发内在驱动(感官刺激降低、积极预期产生)。结论:医护人员应及时评估患者在携带预防性回肠造口期间的症状及管理状况,建立有针对性的干预措施,提高患者在携带预防性回肠造口期间的生活质量。

Objective:To explore the symptoms experienced by patients with rectal cancer while carrying a prophylactic ileostomy.Methods:Through phenomenological research method of qualitative research,using semi-structured interview outline,17 subjects were interviewed in depth.The data were collated and analyzed using Colaizzi method.Results:Four themes were obtained through data analysis.They were the coexistence of complex symptoms(special symptoms of anal and ileostomy,large amount of excretion from the stoma and sleep disturbance),negative self-perceptions(impaired self-esteem,remorse guilt),derived adaptive behavior(seeking supportive information,changing dietary structure),induced internal drive(decreased sensory stimulation,positive expectation).Conclusion:Nursing staff should timely evaluate the symptoms and self-management status of patients during prophylactic ileostomy,and establish targeted intervention measures to improve patients'quality of life during the period of carrying prophylactic ileostomy.
