摘要
目的:比较单次激发屏气磁共振胰胆管成像(magnetic resonance cholangiopancreatography,MRCP)、导航触发的磁共振胰胆管成像(navigator-triggered-mRCP,NT-MRCP)与梯度自旋回波磁共振胰胆管成像(breath-hold(BH)-GraSE-MRCP)的成像质量,并探讨其分别对胆道系统结石的诊断效能。方法:选取2019年6月—2020年12月于荆州市第一人民医院行MRCP检查的罹患胰胆管疾病者74例,按序进行2D-MRCP、NT-MRCP与BH-GraSE-MRCP成像检查,采用配对样本t检验分析三组MRCP序列胰胆管系统显影清晰度、图像伪影及背景抑制的差异性。结果:三组MRCP序列整体图像质量差异无统计学意义(P>0.05)。BH-GraSE-MRCP组对胆囊管的显示情况显著优于NT-MRCP组和2D-MRCP组(P<0.05)。BH-GraSE-MRCP组对右肝管前支、右肝管后支及左肝管内侧支的显示显著优于NT-MRCP组(P<0.05)。NT-MRCP组图像常出现呼吸运动伪影,评分显著低于2D-MRCP组与BH-GraSE-MRCP组(P<0.05)。BH-GraSE-MRCP组的背景抑制效果显著低于2D-MRCP组与NT-MRCP组(P<0.05)。结论:在患者配合情况良好的情况下,2D-MRCP、NT-MRCP与BH-GraSE-MR-CP均能较好地显示胰胆管系统,但BH-GraSEMRCP能更好地显示胆管系统结石,且扫描时间更短。
Objective To compare the imaging quality of 2D-MRCP,NT-MRCP and BH-GraSE-MRCP,and to explore their diagnostic efficacy for stones in biliary system.Methods 74 patients with pancreaticobiliary diseases who underwent MRCP in the first people's Hospital of Jingzhou City from June 2019 to December 2020 were selected.2d-mrcp,nt-mrcp and BH grace-mrcp were performed in sequence,Paired sample t-test was used to analyze the differences of the three MRCP sequences in the definition of pancreaticobiliary system,image artifacts and background suppression.Results There was no significant difference in the quality of images among the three MRCP groups(P﹥0.05).BH-GraSE-MRCP group was significantly superior to NT-MRCP and 2D-MRCP in displaying cystic duct(P﹤0.005).BH-GraSE-MRCP was significantly superior to NT-MRCP in displaying RHD-AB,RHD-PB and LHD-MB,BH-GraSE-MRCP(P﹤0.005).NT-MRCP had more artifacts than BH-GraSE-MRCP and 2D-MRCP(P<0.005).NT-MRCP and 2D-MRCP had more beautiful background suppression than BH-GraSE-MRCP group(P﹤0.005).Conclusion With the cooperation of patients,2D-MRCP,NT-MRCP and BH-GraSE-MRCP can display the pancreaticobiliary system well,Compared with NT-MRCP and 2D-MRCP,BH-GraSE-MRCP can better display stones of pancreaticobiliary system,and the scanning time is shorter.
