2型糖尿病患者胃肠道恶性肿瘤标志物CA199的变化及临床意义

Changes in CA199,a marker for gastrointestinal malignancy,in patients with type 2 diabetes mellitus and its clinical significance

目的观察2型糖尿病患者CA199升高与胃肠道恶性肿瘤的关系。方法将2016年1月至2020年1月阳江市人民医院内分泌科收治的135例2型糖尿病患者分为两组,2型糖尿病合并血清CA199水平升高(CA199≥37 U/ml)的患者为阳性组(n=67),2型糖尿病合并血清CA199水平正常(CA199<37 U/ml)的患者为阴性组(n=68)。收集所有患者年龄、性别、体重、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、肝功能、肾功能以及血脂六项等,探讨2型糖尿病患者血清CA199升高的临床特点与胃肠道恶性肿瘤之间的相关性。结果阳性组HbA1c、FPG、尿微量白蛋白与肌酐比值(UACR)升高,血清白蛋白降低,组间差异具有统计学意义(P<0.05)。Pearson相关性分析显示,CA199与FPG、HbA1c、血清白蛋白相关(r=0.255、0.275、-0.210,P<0.05)。经多元线性回归分析显示,HbA1c和FPG是CA199升高的独立危险因素(β=0.914、0.785,P<0.05)。结论2型糖尿病患者CA199升高与胃肠道恶性肿瘤呈正相关。

Objective To observe the relationship between elevated CA199 and gastrointestinal malignancies in patients with type 2 diabetes mellitus(T2DM).Methods One hundred and thirty-five patients with T2DM admitted to the Department of Endocrinology,People's Hospital of Yangjiang from January 2016 to January 2020 were divided into 2 groups.Patients with T2DM combined with elevated serum CA199 levels(CA199≥37 U/ml)were the positive group(n=67),while patients with T2DM combined with normal serum CA199 levels(CA199<37 U/ml)were the negative group(n=68).Age,gender,weight,fasting plasma glucose(FPG),glycated hemoglobin A1c(HbA1c),liver function,kidney function,and Lipids panel 6 measures were collected from all patients to explore the correlation between the clinical characteristics of elevated serum CA199 and gastrointestinal malignancies in patients with T2DM.Results HbA1c,FPG and urinary microalbumin/creatinine ratio(UACR)were elevated and serum albumin was decreased in the positive group,with statistically significant differences between groups(P<0.05).Pearson correlation analys is s howed that CA 199 correlated with F PG,H bA1c,serum alb umin(r=0.255,0.275,-0.210,P<0.05).By multiple linear regression analysis,HbA1c and FPG were independent risk factors for elevated CA199(β=0.914,0.785,P<0.05).Conclusion Elevated CA199 was positively correlated with gastrointestinal malignancies in patients with T2DM.
