老年慢性肾衰竭行腹膜透析患者导管感染的危险因素

Risk factors for catheter infection in elderly patients with chronic renal failure undergoing peritoneal dialysis

目的探讨老年慢性肾衰竭(CRF)行腹膜透析患者导管感染的危险因素。方法选取2017年10月至2018年10月湖北省中医院收治的老年CRF患者40例作为研究对象。通过问卷调查的方式,详细记录患者基本资料及导管感染现状。单因素分析影响老年CRF行腹膜透析患者导管感染的相关危险因素,通过多因素Logistic回归分析影响老年CRF行腹膜透析患者导管感染的独立危险因素。结果本研究调查结果显示,该院收治的40例老年CRF行腹膜透析患者中,9例患者发生导管感染,占比22.5%;未感染的患者31例,占77.5%;年龄、高血压、糖尿病、透析时间及防护不规范是影响老年CRF行腹膜透析患者导管感染的独立危险因素(OR>1,P<0.05)。结论本研究选取的老年CRF行腹膜透析患者导管感染发生率为22.5%,导管感染率较高;年龄、糖尿病、透析时间及防护不规范是影响老年CRF行腹膜透析患者导管感染的独立危险因素。临床医务人员应针对以上风险因素,采取相应护理措施,以改善患者营养状况、血糖及血压等水平,提升患者规范防护的认知水平及操作技能,对于高龄、透析时间较长及合并其基础疾病的患者予以重点关注,从而降低导管感染的风险。

Objective To investigate the risk factors of catheter infection in elderly patients with chronic renal failure(CRF)undergoing peritoneal dialysis.Methods Forty elderly patients with CRF admitted to Hubei Provincial Hospital of Traditional Chinese Medicine from October 2017 to October 2018 were selected as the study subjects.The basic information of patients and the status of catheter infection were recorded in detail by means of questionnaire survey.Univariate analysis was used to analyze the related risk factors of catheter infection in elderly CRF patients undergoing peritoneal dialysis,and multivariate Logistic regression was used to analyze the independent risk factors of catheter infection in elderly CRF patients undergoing peritoneal dialysis.Results The results of this study showed that among 40 elderly patients with CRF who underwent peritoneal dialysis admitted to this hospital,9 patients developed catheter infection,accounting for 22.5%,and 31 patients were not infected,accounting for 77.5%.Age,hypertension,diabetes,dialysis duration and non-standard protection were independent risk factors for catheter infection in elderly CRF patients undergoing peritoneal dialysis(OR>1,P<0.05).Conclusions In this study,the incidence of catheter infection in elderly CRF patients undergoing peritoneal dialysis was 22.5%,with a high catheter infection rate.Age,diabetes,duration of dialysis,and nonstandard protection are independent risk factors for catheter infection in elderly CRF patients undergoing peritoneal dialysis.Clinical medical personnel should be aimed at the above risk factors and take corresponding nursing measures to improve the nutritional status of patients with blood sugar and blood pressure levels,improve patients with standard protective cognition level and operation skill,for age,dialysis for a long time,and its basic diseases combined to focus on,so as to reduce the risk of catheter infection.
