新生儿红斑狼疮15例临床分析及文献回顾

目的探讨新生儿红斑狼疮的临床特点并进行文献回顾。方法回顾性分析徐州市儿童医院15例新生儿红斑狼疮患儿的临床资料,对新生儿红斑狼疮的临床特点进行分析总结,并进行随访。结果 15例患儿中女11例,男4例,就诊年龄8 d~3个月。皮损位于头面部14例,位于非曝光部位8例;皮损表现为水肿性红斑样13例,表现为脂溢性皮炎2例;并发房室传导阻滞1例;4例患儿母亲患有干燥综合征,无临床症状11例。抗核抗体比值1:100~1:1 600,所有患儿SSA和(或)SSB阳性。结论新生儿红斑狼疮具有表型变异性,遇到可疑患儿及时进行心电图及自身免疫抗体检测,确诊后应长期随访。

Objective To investigate the clinical characteristics of neonatal lupus erythematosus(NLE), so as to enforce the knowledge of the disease. Methods The clinical and photographic data of 15 cases of NLE were studied retrospectively and the clinical characteristics of NLE were analyzed and summarized, as well as the related literatures were reviewed. Results For the 15 cases of NLE, 11 were females and 4 was males, and the minimum age was 8 days and the maximum was 3 months. There were 14 patients whose skin lesions were localized on the scalp and face, 8 patients with the skin lesions in non-sun exposed areas, 13 cases manifested as erythema annulare and 2 cases as seborrheic dermatitis, and 1 case with severe atrioventricular block. For the mothers of the 15 patients, four were with Sjogren’s syndrome and 11 without clinical symptoms. The ratio of ANA ranged from 1:100 to 1:1600, and all the cases were positive for anti-Ro/SSA and/or anti-LA/SSB. Conclusion There is phenotypic variability of NLE. For the suspicious cases, electrocardiogram and autoimmune antibody should be detected timely, and long-term follow-up should be conducted after diagnosis.
