机械刺激诱发鼻部皮肤假性淋巴瘤一例

A case of nasal cutaneous pseudolymphoma induced by mechanical stimulation

47岁女性患者,鼻旁暗红色肿物伴瘙痒2个月余。皮肤科情况:左鼻翼单发蚕豆大小暗红色肿物,表面光滑,境界清楚,触之柔韧。组织病理示:真皮内可见淋巴细胞结节状浸润。免疫组化染色示CD2、CD3、CD20(L26)均阳性,CD4、CD8、CD21、PD-1、KP-1散在阳性,Ki-67(MIB-1)7%阳性。诊断:T/B混合型结节状皮肤假性淋巴瘤。

A 47-year-old female presented with a dark red pruritic mass beside the nose for more than 2 months. Dermatological examination showed a solitary, hemispherical mass with dark red color on left nasal alar. The mass about broad bean size was well defined, with smooth surface and no tenderness. Histopathological examination showed that the epidermis was roughly normal, and there was a nodular infiltration of lymphocytes in the dermis which also involved the appendages and striated muscle. Immunohistochemical staining showed a mixed infiltration of T and B cells: CD2(+), CD3(+), CD4(+), CD8(+), CD20(L26)(+), CD21(partly +), PD-1(+), Ki-67(MIB-1)(7%+), KP-1(a few +). The diagnosis of nodular T/B-cell mixed cutaneous pseudolymphoma was established.
