摘要
12岁男性患儿,头皮孤立性结节12年。皮肤科情况:头顶部可见一直径0.5 cm大小结节,表面光滑,触之有囊性感。皮损组织病理示:表皮轻度角化过度,颗粒层略增厚,棘细胞层增厚;真皮浅层散在以淋巴细胞为主的炎性细胞浸润,真皮中下层可见较多大汗腺腺腔样结构,腔壁由单层立方形细胞组成,见顶浆分泌,腺腔内为嗜碱性无结构物质,囊腔周围纤维组织增生。诊断:大汗腺汗囊瘤。
The patient was a 12-year-old male with an isolated nodule on the scalp for 12 years.Specialist physical examination:a nodule about 0.5cm in diameter located on the scalp with smooth surface and cystic touch feeling.Surgical removal was performed delicately.Histopathological examination(HE and PAS staining)showed that the epidermis was slight hy perkeratosis,with slight hyperplasia of the stratum granulosum and the spinous cell layer;inflammatory infiltration in the superficial dermis,mainly composed of lymphocytes;more apocrine sweat gland cave-like structures in the middle and deep dermis,lined by one layer of cuboidal cells with the features of apocrine secretion,with non-structural basophilic substances in the gland cavity;collagen fibrous tissue hyperplasia.Diagnosis:apocrine hidrocystoma.
