口腔癌游离皮瓣修复术后血红蛋白、白蛋白及前白蛋白水平分析 被引量:7

Analysis of postoperative variation of blood protein in patients with oral cancer

摘要 目的:分析口腔癌游离皮瓣修复术患者血红蛋白、白蛋白以及前白蛋白水平变化。方法:回顾性分析40例口腔癌患者的血常规及血生化指标,分析记录入院时、手术前1天、术后第1天、术后第3天、术后第5天、术后第7天以及出院当天血液血红蛋白、白蛋白以及前白蛋白水平变化。结果:和术前相比,3项蛋白指标术后第1天均开始下降,术后第3天下降至最低。双侧颈清组比单侧颈清组下降幅度更大(P<0.05),腓骨瓣组相对于股前外皮瓣组下降幅度更大(P<0.05)。结论:口腔癌游离皮瓣修复术后患者前3天血红蛋白、白蛋白以及前白蛋白水平均持续下降,行双侧颈淋巴清扫术及腓骨瓣移植患者下降更为明显,在此期间患者需更多的营养支持。 Objective:To investigate the postoperative variation of blood protein in patients with oral cancer.Methods:Forty patients with oral cancer who were treated in our hospital were enrolled in this retrospective analysis study.The changes of hemoglobin, plasma albumin, and preablumin of all patients on the day hospitalized, the day before operation, 1 days, 3 days, 5 days, 7 days after operation and the last day in hospital were observed.The differences between the groups divided by the type of neck dissection and skin flap were discussed.Results:The protein level began to decline after operation and reached the lowest on the 3 days after operation.The bilateral neck dissection group decreased significantly compared with the unilateral group(P<0.05).The fibular flap group decreased significantly compared with the anterolateral thigh flap group(P<0.05).Conclusion:The blood protein level continues to decrease for the first 3 days after operation, especially in patients with bilateral neck dissection and fibular flap surgery.During this period, more nutritional support is needed.
