患者参与式饮食干预对老年肺癌化疗患者营养状态和疲乏的改善效果

Effects of patient participation-based dietary intervention on nutritional status and cancer-related fatigue in elderly patients with lung cancer undergoing chemotherapy

目的探讨患者参与式饮食干预(patient participation-based dietary intervention,PPDI)对老年肺癌化疗患者营养状态和疲乏的改善效果。方法选择接受化疗的老年非小细胞肺癌患者90例,随机分为试验组和对照组,每组45例。对照组采用常规化疗期护理,试验组在常规护理基础上实施患者参与式饮食干预。于入院时和化疗后,采用癌症疲乏量表(cancer fatigue scale,CFS)、患者整体营养主观评估量表(patient-generated subjective global assessment,PG-SGA)和体格测量指标评价干预效果。结果入院时,2组患者营养指标、CFS量表得分差异无统计学意义(P>0.05);化疗后,试验组体重指数(body mass index,BMI)、上臂肌围(arm muscle circumference,AMC)明显低于对照组,PG-SGA得分明显低于对照组;试验组躯体疲乏、情感疲乏、认知疲乏和疲乏总分明显低于对照组,差异均有统计学意义(P<0.05)。结论患者参与式饮食干预能有效改善老年肺癌化疗患者营养状态,缓解疲乏症状。

Objective To investigate the improvement effects of patient participation-based dietary intervention(PPDI)on nutritional status and cancer-related fatigue in elderly patients with lung cancer undergoing chemotherapy.Methods A total of 90 elderly patients with lung cancer who underwent chemotherapy were randomly divided into experimental group and control group,with 45 cases in each group.The patients in control group were treated by routine nursing,however,the patients in experimental group,on the basi of control group,were treated by patient participation-based dietary intervention.On admission and after chemotherapy,the intervention effects was assessed by Cancer fatigue scale(CFS)and patient-generated subjective global assessment(PG-SGA)as well as physical examination index.Results On admission,there were no significant differences in nutrition indexes and CFS scores between the two groups(P>0.05).After chemotherapy,the body mass index(BMI),arm muscle circumference(AMC)in experimental group were significantly lower than those in control group,moreover,the PG-SGA scores,the physical fatigue scores,emotional fatigue scores,cognitive fatigue scores and total fatigue scores were significantly lower than those in control group(P<0.05).Conclusion The patient participation-based dietary intervention can effectively improve the nutritional status and alleviate cancer-related fatigue in elderly patients with lung cancer after chemotherapy.
