婴幼儿轮状病毒肠炎致肝损害的高危因素探讨 被引量:1

Analysis the Risk Factors of Liver Damage Induced by Rotavirus Enteritis in Infants

摘要 目的探讨轮状病毒(rotavirus, RV)肠炎患儿致肝损害的临床特点及高危因素。方法选取作者医院2015-01/2019-12月收治住院的60例RV肠炎患儿作为观察组,同期住院的非RV肠炎患儿60例为对照组,测定两组患儿血清中天门冬氨酸氨基转移酶(aspartate aminotransferase, AST)及丙氨酸氨基转移酶(alanine aminotransferase, ALT)表达水平,采用单因素及多因素logistic回归分析RV肠炎合并肝功能损害的高危因素。结果观察组患儿血清中ALT及AST水平显著高于对照组患儿(P<0.05)。在单因素分析中RV感染在呕吐、脱水、酸中毒影响因素中差异具有统计学意义(P<0.05),而多因素分析显示仅脱水具有统计学意义(P<0.05)。结论临床上需注意RV肠炎患儿引起的肝功能异常,且脱水患儿更易发生肝功能损害。 Objective To explore the clinical characteristics and risk factors of liver damage induced by rotavirus(RV) enteritis. Methods A total of 60 infants of RV enteritis hospitalized in author′s hospital from January 2015 to December 2019 were selected as the observation group, sixty infants of non-RV enteritis in the same period were selected as the control group. The levels of serum alanine aminotransferase(AST) and aspartate aminotransferase(ALT) of infants were analyzed.The risk factors of RV enteritis complicated with liver dysfunction were analyzed by univariate analysis and multivariate logistic regression. Results The levels of serum ALT and AST of infants in observation group were significantly higher than those of the control group(P<0.05). The univariate analysis showed that, the RV infection in vomiting, dehydration, acidosis were significantly differences(P<0.05), while multivariate analysis showed that only dehydration was significantly difference(P<0.05). Conclusion Clinical attention should be paid to abnormal liver function caused by infants with RV enteritis, and the infants with dehydration are more likely to have liver dysfunction.
