基于数据挖掘的刘伟胜治疗大肠癌用药规律

Study on LIU Weisheng’s Medication Rule in Treating Colorectal Cancer Based on Data Mining

目的:运用数据挖掘技术对刘伟胜教授治疗大肠癌的临床用药规律进行分析。方法:运用古今医案云平台(V2.2.2),对2014年1月至2019年11月于刘伟胜教授门诊就诊的大肠癌患者用药进行药物频次统计、四气五味和归经分析、药物关联度分析、聚类分析、复杂网络分析。结果:研究共纳入处方205首,涉及中药151味,药性以平、温、微寒及甘、苦、辛味为主,多归脾、肝、肺、肾经。高频中药频次前3位为茯苓、全蝎、党参,高频药对及关联规则分析前三位为“党参-茯苓”“全蝎-茯苓”“党参-全蝎”,聚类分析得出4类组合药物,复杂网络分析挖掘药物核心组合为党参、茯苓、柴胡、白芍、女贞子、白术、砂仁、全蝎、半枝莲、白花蛇舌草、甘草。结论:刘伟胜教授治疗大肠癌常用健脾与疏肝并举的治法,通过辨病与辨证相结合,在益气扶正、解毒抗癌的同时注重疏畅气机,体现了平调气机、以通为宜的整体指导思想。

Objective:To analyze the clinical medication rule of Professor LIU Weisheng in treating colorectal cancer by applying data mining teconology.Methods:By using Ancient and Modern Medical Case Cloud Platform(V2.2.2),the drug frequency statistics,four qi and five tastes meridian analysis,drug association rule analysis,cluster analysis,complex network analysis of colorectal cancer patients treated in Professor LIU Weisheng’s outpatient clinic from January 2014 to November 2019 were analyzed.Results:A total of 205 prescriptions were included,involving 151 herbs.The main properties of the herbs were even,warm and slightly cold,and the main tastes were sweet,bitter and pungent.The main meridians were the spleen,liver,lung and kidney meridian.The top three high-frequency herbs were Fuling(Poria),Quanxie(Scorpio),Danshen(Codonopsis Radix).The top three high-frequency drug pairs and correlation analysis were“Dangshen-Fuling”,“Quanxie-Fuling”and“Dangshen-Quanxie”.Cluster analysis found 4 types of combination drugs.The core prescription analyzed by complex network included Dangshen,Fuling,Chaihu(Bupleuri Radix),Baishao(Paeoniae Radix Alba),Nvzhenzi(Ligustri Lucidi Fructus),Baizhu(Atractylodis Macrocephalae Rhizoma),Sharen(Amomi Fructus),Quanxie,Banzhilian(Scutellariae Barbatae Herba),Baihuasheshecao(Herba Hedyotidis),Gancao(Glycyrrhizae Radix Et Rhizoma).Conclusions:In the treatment of colorectal cancer,Professor LIU Weisheng applies the method of invigorating the spleen and soothing the liver at the same time.Through the combination of disease differentiation and syndrome differentiation,he pays attention to relieving qi while strengthening qi,detoxifying and anticancer,which embodies the overall guiding ideology of leveling qi and regulating qi,and the appropriateness of regulation.
