摘要
目的探讨二甲双胍调节肥胖型多囊卵巢综合征(PCOS)患者内分泌及糖脂代谢参数的疗效。方法纳选项城市项城市计划生育技术站2020年1月至2021年3月收诊的肥胖型PCOS患者共计88例,根据信封法分设两组,对照组(n=44例,炔雌醇环丙酮治疗),研究组(n=44例,二甲双胍治疗)。分析两组用药前后糖脂代谢相关指标[低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)、总胆固醇(TC)、胰岛素抵抗指数(HOMA-IR)、空腹血糖(FPG)]和内分泌相关指标[内脂素、卵泡刺激素(FSH)、血清睾酮(T)、黄体生成素(LH)]变化水平及排卵率。结果研究组HDL-C、LDL-C、TC、TG水平低于对照组同期及本组用药前(P<0.05),研究组用药前FSH、T、LH及内脂素水平低于对照组同期及本组用药前(P<0.05)。研究组HOMA-IR水平低于对照组同期及本组用药前(P<0.05);研究组用药前后FPG水平与对照组差异无统计学意义(P>0.05)。用药后,研究组排卵率高于对照组(P<0.05)。结论二甲双胍调节肥胖型PCOS患者内分泌及糖脂代谢参数效果显著,可提升排卵率,值得临床推广。
Objective To investigate the efficacy of metformin in regulating endocrine and glucose and glucose and lipid metabolism in patients with obese polycystic ovary syndrome(PCOS).Methods A total of 88 obese PCOS patients were admitted to Xiangcheng Family Planning Technology Station from January 2020 to March 2021.The patients were divided into two groups by envelope method,with control group(n=44,ethinylestradiol cycloacetone treatment)and study group(n=44 cases,metformin treatment).Change levels of glucose-lipid metabolism[low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),triglycerides(TG),total cholesterol(TC),insulin resistance index(HOMA-IR),fasting blood glucose(FPG)]and endocrine related indicators[endolipoprotein,follicle stimulating hormone(FSH),serum testosterone(T),luteogenesis(LH)]and ovulation rates in two groups were analyzed.Results The levels of HDL-C,LDL-C,TC and TG in the study group were lower than those in the control group at the same period and before medication(P<0.05),and the levels of FSH,T,LH and endolipid in the study group were lower than those in the control group at the same period and before medication(P<0.05).HOMA-IR level of the study group was lower than that of the control group at the same period and before treatment(P<0.05).There was no statistical difference between the FPG levels and the control group(P>0.05).After the medication,the ovulation rate in the study group were higher than those in the control group(P<0.05).Conclusion Metformin has a significant effect on regulating endocrine and glucose and lipid metabolism parameters in obese PCOS patients,and can improve ovulation rate,which is worthy of clinical promotion.
