宫颈癌根治术麻醉中使用右美托咪定的安全性和有效性研究

Safety and Efficacy of Dezocine in Remifentanil Combined Anesthesia for Radical Hysterectomy

目的:探讨地佐辛联合右美托咪定在宫颈癌根治术麻醉中的安全性和有效性。方法:选取2018年5月-2020年9月本科室收治的宫颈癌根治术患者66例,运用奇偶法原则分为常规组和观察组,每组33例。观察组于手术结束前30 min给予地佐辛+右美托咪定,常规组于手术结束前30 min单纯运用地佐辛,对两组效果进行对比。结果:观察组术后1 h、6 h VAS评分低于常规组,差异有统计学意义(P<0.05);观察组拔管时间、苏醒用时及自主呼吸恢复用时短于常规组,差异有统计学意义(P<0.05);两组不良反应总发生率及术后痛觉过敏率比较,差异无统计学意义(P>0.05)。结论:宫颈癌根治术患者瑞芬太尼复合麻醉中采取地佐辛联合右美托咪定可减轻术后痛觉过敏症状,同时有益于缩短苏醒时间,且不增加不良反应发生概率,应用安全性较高。

Objective: To investigate the clinical preventive effect of dezocine on postoperative hyperalgesia in patients undergoing radical cervical cancer surgery with remifentanil combined anesthesia. Methods: Selected 66 patients undergoing radical cervical cancer surgery from May 2018 to September 2020. and they were grouped using the principle of parity, namely the routine group(n = 33) and the observation group(n = 33). The observation group was given dezocine+dexmedetomidine 30 min before the end of the operation, and the conventional group was given dezocine only 30 min before the end of the operation. The effects of the two groups were compared. Results: The VAS scores at 1 h and 6 h after operation in the observation group were lower than those in the conventional group, and the difference was statistically significant(P < 0.05);The extubation time, recovery time and spontaneous breathing recovery time of the observation group were shorter than those of the conventional group, and the difference was statistically significant(P < 0.05);There was no significant difference in the total incidence of adverse reactions and postoperative hyperalgesia between the two groups(P > 0.05). Conclusion: Dezocine combined with remifentanil combined anesthesia for patients undergoing radical cervical cancer surgery can reduce postoperative hyperalgesia symptoms, and at the same time, it is beneficial to shorten the recovery time, has fewer adverse reactions, and has high application safety. It is worthy of promotion.
