摘要
目的探讨加温湿化高流量鼻导管通气治疗婴幼儿重症肺炎的效果及对血气分析指标的影响。方法选择2019年9月—2020年9月收治的80例婴幼儿重症肺炎患儿为研究对象,随机数表法分为对照组与观察组,各40例,两组均接受常规药物治,对照组采用无创正压通气,观察组采用加温湿化高流量鼻导管通气,对两组治疗效果及血气分析指标进行比较。结果观察组治疗总有效率为90.00%,较对照组67.50%高,差异有统计学意义(χ^(2)=6.050,P<0.05);两组治疗前的动脉氧分压(PaO_(2))、动脉二氧化碳分压(PaCO_(2))、血氧饱和度(SaO_(2))差异无统计学意义(P>0.05),观察组治疗后PaO_(2)、SaO_(2)较对照组高,PaCO_(2)较对照组低,差异有统计学意义(P<0.05)。结论加温湿化高流量鼻导管通气用于婴幼儿重症肺炎中效果较好,可促进血气分析指标改善,值得推广。
Objective To investigate the effect of warming,humidifying,high-flow nasal catheter ventilation in the treatment of infants and young children with severe pneumonia and its influence on blood gas analysis indexes.Methods Eighty infants with severe pneumonia admitted from September 2019 to September 2020 were selected as the research objects.Random number table method was used to divide the patients into the control group and the observation group with 40 cases each.Both groups received conventional drug treatment.The control group used non-invasive positive pressure ventilation,and the observation group used warm-humidified high-flow nasal cannula ventilation.The therapeutic effects and blood gas analysis indexes of the two groups were compared.Results The total effective rate of treatment in the observation group was 90.00%,which was higher than 67.50%in the control group,and the difference was statistically significant(χ^(2)=6.050,P<0.05);the arterial partial pressure of oxygen(PaO_(2)),arterial carbon dioxide(PaCO_(2)),blood pressure before treatment in the two groups had no statistically significant difference in oxygen saturation(SaO_(2))(P>0.05).After treatment,PaO_(2) and SaO_(2) in the observation group were higher than those in the control group,and PaCO_(2) was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Warming and humidifying high-flow nasal catheter ventilation is effective for severe pneumonia in infants and young children.It can promote the improvement of blood gas analysis indicators and is worthy of promotion.
