腹腔镜和开腹子宫肌瘤剔除术治疗子宫肌瘤临床疗效评价

Evaluation of the Clinical Efficacy of Laparoscopic and Open Uterine Myomectomy in the Treatment of Uterine Fibroids

目的探讨对子宫肌瘤行传统开腹手术与腹腔镜子宫肌瘤剔除术治疗的效果差异。方法选择该院2017年1月—2019年12月收治的子宫肌瘤患者80例,按照随机数表法分为开腹手术治疗的对照组(n=40)与采用腹腔镜子宫肌瘤剔除术治疗的研究组(n=40),对比手术相关指标及术后恢复情况。结果两组均顺利完成手术,手术时间差异无统计学意义(P>0.05)。研究组术中出血量为(57.81±15.73)mL,小于对照组(105.73±16.52)mL,差异有统计学意义(t=13.286,P<0.05),剔除肌瘤个数为(2.21±0.40)个,多于对照组(1.48±0.31)个,差异有统计学意义(t=9.123,P<0.05)。研究组术后排气时间、体温恢复时间、住院时间与复发率分别为(1.13±0.30)d、(1.12±0.43)d、(5.13±0.65)d、0.00%(0/40),均小于对照组,差异有统计学意义(t=9.446、7.976、12.016,χ^(2)=4.505,P<0.05)。结论对子宫肌瘤患者采用腹腔镜子宫肌瘤剔除术治疗效果理想,有利于肌瘤的完全剔除,对患者的损伤小,患者术后康复快且复发率低,值得广泛应用。

Objective To explore the difference between traditional open surgery and laparoscopic myomectomy for uterine fibroids.Methods Chose the hospital from January 2017 to December 2019 to treat 80 patients with uterine fibroids.According to the random number table,they were divided into control group(n=40)treated by open surgery and the study group(n=40)treated bylaparoscopic myomectomy,the operation-related indicators and postoperative recovery were compared.Results Both groups completed the operation successfully,and there was no statistically significant difference in operation time(P>0.05).The intraoperative blood loss of the study group was(57.81±15.73)mL,which was smaller than the control group(105.73±16.52)mL,the difference was statistically significant(t=13.286,P<0.05),and the number of fibroids eliminated was(2.21±0.40),which was more than the control group(1.48±0.31),the difference was statistically significant(t=9.123,P<0.05).The postoperative exhaust time,body temperature recovery time,hospitalization time and recurrence rate of the study group were(1.13±0.30)d,(1.12±0.43)d,(5.13±0.65)d,and 0.00%(0/40),respectively,which were all less than control group,the difference was statistically significant(t=9.446,7.976,12.016,χ^(2)=4.505,P<0.05).Conclusion Laparoscopic myomectomy for patients with uterine fibroids has an ideal therapeutic effect.It is conducive to the complete elimination of fibroids,with little damage to patients,rapid recovery after surgery and low recurrence rate,and it is worthy of wide application.
