超声心动图联合心电图对AHCM患者诊断准确性的影响 被引量:1

The Influence of Echocardiography Combined with Electrocardiogram on the Diagnostic Accuracy of AHCM Patients

摘要 目的以心尖肥厚型心肌病(AHCM)者为判定对象,观察分析超声心动图与心电图联合诊断检查的效果情况。方法选取2018年1月—2020年12月该院接收的62例经病理诊断确诊的AHCM患者作为观察组,同期另选取行健康检查者62名作为对照组。对研究所选对象均行心电图检查与超声心动图检查,分析两种检查方式共同应用的效果情况。结果超声心电图检查指标比较,较对照组,观察组心尖厚度和室间隔厚度(IVS)、左心房内径(LAD)和左心室后壁厚度(LVPW)、心尖厚度/左心室后壁厚度和E/A数据结果更高,A峰、E峰数据结果更低,差异有统计学意义(P<0.05)。心电图导联不同R波振幅增高幅度比较,较对照组,观察组数据结果更高,差异有统计学意义(P<0.05)。心电图导联不同ST段下移幅度比较,较对照组,观察组数据结果更高,差异有统计学意义(P<0.05)。心肌肥厚指标比较,对照组的S_(v3)+R_(aVL)(1.15±0.44)、ΣQRS(13.48±3.27)、R_(v3)+S_(v1)(1.83±0.54)、R_(v2)+S_(v1)(1.36±0.36)、S_(v1)+R_(v5)(2.18±0.73)mV均低于观察组的(1.63±0.83)、(21.53±7.38)、(3.35±1.26)、(2.67±0.65)、(4.15±1.06)mV,差异有统计学意义(t=4.023、7.853、8.731、13.882、12.052,P<0.05)。结论在临床诊断心尖肥厚型心肌病患者中,予以患者行心电图与超声心动图共同检查,能取得较好的诊断效果,可有效诊断鉴别患者的心肌肥厚与左心功能情况,同时还能对其肥厚区域与心脏形态情况有效观察,能使诊断准确率提升,为临床治疗方案制定、病情程度判断提供依据支持。 Objective For apical hypertrophic cardiomyopathy(AHCM)patients,to observe and analyze the effect of combined diagnosis of echocardiography and electrocardiography.Methods The 62 patients with AHCM diagnosed by pathology diagnosed in the hospital from January 2018 to December 2020 were selected as the observation group,and another 62 patients who underwent health examination were selected as the control group during the same period.Both ECG and echocardiography were performed on the subjects selected in the study,analyzed the effect of the joint application of the two inspection methods.Results Compared with the control group,the observation group’s apical thickness and IVS,LAD and LVPW,apical thickness/posterior left ventricular wall thickness,and E/A data were higher than those of the control group,while the A peak and E peak data scores were lower.The difference was statistically significant(P<0.05).Compared the increase in R wave amplitude of different ECG leads,the data of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Compared the different ST-segment descending amplitudes of ECG leads,the data of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Comparison of myocardial hypertrophy indicators,the control group's S_(v3)+R_(aVL)(1.15±0.44)mV,ΣQRS(13.48±3.27)mV,R_(v3)+S_(v1)(1.83±0.54)mV,R_(v2)+S_(v1)(1.36±0.36)mV,S_(v1)+R_(v5)(2.18±0.73)mV,all were lower than the observation group(1.63±0.83)mV,(21.53±7.38)mV,(3.35±1.26)mV,(2.67±0.65)mV,(4.15±1.06)mV,the difference was statistically significant(t=4.023,7.853,8.731,13.882,12.052,P<0.05).Conclusion In the clinical diagnosis of patients with apical hypertrophic cardiomyopathy,the combined examination of electrocardiogram and echocardiography can achieve better diagnostic results,which can effectively diagnose and distinguish the patients’myocardial hypertrophy and left ventricular function.Effective observation of hypertrophic areas and cardiac morphology can improve the accuracy of diagnosis,and provide basis support for the formulation of clinical treatment plans and the judgment of disease severity.
