腹股沟疝无张力修补术后补片感染的诊疗效果研究

Study on the Diagnosis and Treatment Effect of Patch Infection after Tension-free Inguinal Hernia Repair

目的分析腹股沟疝无张力修补术后补片感染的诊疗效果。方法研究分析该院2018年4月—2020年3月收治的60例腹股沟疝无张力修补术后补片感染患者,以数字法对患者随机分组,每组30例,一组应用清创手术治疗,另一组在此基础上应用伤口Ⅰ期缝合治疗,分别作为对照组和观察组,对比两组的住院时间、抗生素使用时间和伤口愈合情况、并发症。结果观察组患者住院时间是(7.34±2.76)d,与对照组对比差异无统计学意义(t=0.427,P>0.05),其抗生素使用时间是(4.22±1.65)d,伤口甲级愈合93.33%(28例),乙级愈合6.67%(2例),数据优于对照组,差异有统计学意义(t=5.189,χ^(2)=12.273,P<0.05);观察组并发症发生率是10.00%(3例),对照组是40%(12例),组间差异有统计学意义(χ^(2)=7.200,P<0.05)。结论腹股沟疝无张力修补术后补片感染应用清创+伤口Ⅰ期缝合治疗效果较好,可减少抗生素的使用,促进伤口预后,术后并发症较少。

Objective To analyze the diagnosis and treatment effect of patch infection after tension-free inguinal hernia repair.Methods A total of 60 patients with patch infection after tension-free repair of inguinal hernia admitted to our hospital from April 2018 to March 2020 were analyzed.Patients were randomly divided into two groups by digital method,30 cases in each group one group was treated with debridement surgery,and the other group was treated withⅠstage suture on the basis of this,respectively,as the control group and the observation group.The length of hospital stay,duration of antibiotic use,wound healing and complications were compared between the two groups.Results The hospital stay of the observation group was(7.34±2.76)d,and the difference was not statistically significant compared with the control group(t=0.427,P>0.05).The duration of antibiotic use was(4.22±1.65)d.Grade A wound healing was 93.33%(28 cases)and grade B wound healing was 6.67%(2 cases),which were better than that of the control group.The difference was statistically significant(t=5.189,χ^(2)=12.273,P<0.05).The incidence of complications was 10.00%(3 cases)in the observation group and 40%(12 cases)in the control group,and the difference between groups was statistically significant(χ^(2)=7.200,P<0.05).Conclusion After the tension-free repair of the inguinal hernia,the application of patch infection with debridement+first-stage wound suture is better.It can reduce the use of antibiotics,promote the prognosis of the wound,and has fewer postoperative complications.
