无痛分娩技术在产妇自然分娩中的应用及缩短产程时间分析

Application of Painless Delivery Technology in Natural Delivery of Parturients and Analysis of Shortening the Time of Labor

目的研究无痛分娩技术在产妇自然分娩中的应用效果以及对缩短产妇产程时间的影响。方法在该院2019年8月—2020年8月期间收治产妇中,随机选出200名自然分娩的产妇作为研究对象,通过随机表数字法将其分为对照组(n=100)和研究组(n=100),两组产妇分别安排应用传统自然分娩和无痛分娩技术,比较其新生儿1 min Apgar评分、产妇产后2 h出血量、产后24 h血清泌乳素、会阴裂伤程度以及产程时间差异。结果研究组产后2 h出血量(94.05±16.68)mL低于对照组(120.02±18.84)mL,产后24 h血清泌乳素(330.11±42.75)ng/mL高于对照组(288.48±45.56)ng/mL,差异有统计学意义(t=10.321、6.663,P<0.05);研究组会阴完整率(20.00%)、裂伤I度情况(69.00%)优于对照组(5.00%、55.00%),裂伤II度情况(10.00%)低于对照组(36.00%),对照组明显比研究组更为严重;研究组第一、二、三产程时间(9.86±0.97)h、(0.89±0.20)h、(8.41±1.22)min短于对照组,差异有统计学意义(t=13.243、7.605、2.681,P<0.05)。结论在选择自然分娩的产妇群体中,无痛分娩技术具有良好的意义价值,不仅对其产后泌乳功能具有良好的改善作用,而且还可以在减少产后出血量的同时,进一步缩短产妇产程时间。

Objective To study the application effect of painless delivery technique in the natural delivery of parturients and the effect on shortening the time of parturient process.Methods In the hospital from August 2019 to August 2020,200 cases of parturients who gave birth naturally were randomly selected as the research objects,and they were divided into the control group(n=100)and the study group(n=100)by the random table number method,the two groups of parturients were arranged to apply traditional natural delivery and painless delivery respectively to compare their newborn 1 min Apgar score,parturients 2 h postpartum hemorrhage,24 h postpartum serum prolactin,degree of perineal laceration,and labor time.Results The postpartum blood loss(94.05±16.68)mL of the study group was lower than that of the control group at(120.02±18.84)mL,and the serum prolactin(330.11±42.75)ng/mL at 24 h postpartum was higher than that of the control group at(288.48±45.56)ng/mL.The difference was statistically significant(t=10.321,6.663,P<0.05);the intact rate of the perineum in the study group was(20.00%),and the I degree laceration(69.00%)were better than that of the control group by 5.00%and 55.00%,and the degree of laceration II(10.00%)was lower than the control group(36.00%),the control group was significantly more serious than the study group;the first,second,and third stages of labor in the study group(9.86±0.97)h,(0.89±0.20)h,(8.41±1.22)min were shorter than the control group,the difference was statistically significant(t=13.243,7.605,2.681,P<0.05).Conclusion In the group of parturients who choose natural childbirth,the painless delivery technique has good value.It not only has a good effect on improving postpartum lactation function,but also can reduce the amount of postpartum hemorrhage and further shorten the delivery time of the parturient.
