摘要
目的分析并探究超急性脑梗死采取高压氧+阿替普酶+依达拉奉诊疗的效果。方法以2018年10月—2020年10月该科室接诊的70例超急性脑梗死患者为探究对象,以奇偶法原则展开分组,可划分为参照组(n=35)和研究组(n=35),接受阿替普酶+依达拉奉诊疗者纳入参照组,以此为基准,采取高压氧诊疗者纳入研究组,对比两组效果,如有效率、神经功能受损评分等。结果研究组有效率97.14%,与参照组77.14%比较,差异有统计学意义(χ^(2)=4.590,P=0.032);研究组诊疗后7、14 d美国国立卫生院神经功能缺损评分(NIHSS)评分(5.76±1.05)分、(3.79±0.87)分,低于参照组(8.41±2.34)分、(7.15±1.24)分,差异有统计学意义(t=6.113、13.123,P<0.001);研究组炎性因子水平低于参照组,差异有统计学意义(P<0.05);与参照组比,研究组全血黏度、血浆黏度及红细胞容积显著较低,差异有统计学意义(t=4.460、5.445、8.117,P<0.001);两组不良反应比较,差异无统计学意义(P>0.05)。结论超急性脑梗死采取高压氧+阿替普酶+依达拉奉诊疗理想,值得推广。
Objective To analyze and explore the effect of hyperbaric oxygen+alteplase+edaravone diagnosis and treatment of hyperacute cerebral infarction.Methods The 70 patients with hyperacute cerebral infarction received in this department from October 2018 to October 2020 were the subjects of investigation,grouping them according to the principle of odd-odd and dividing them into reference group(n=35)and the study group(n=35),those who received alteplase+edaravone treatment were included in the reference group.Based on this,those who took hyperbaric oxygen treatment were included in the study group,and the effects of the two groups were compared,such as effectiveness,neurological impairment score,etc.Results The effective rate of the study group was 97.14%,compared with 77.14%of the reference group,the difference was statistically significant(χ^(2)=4.590,P=0.032);7 d and 14 d after the study group's diagnosis and treatment,the National Institutes of Health Neurological Impairment Score(NIHSS)was(5.76±1.05)points and(3.79±0.87)points,which were lower than the reference group(8.41±2.34)points and(7.15±1.24)points,the difference was statistically significant(t=6.113,13.123,P<0.001).The level of inflammatory factors in the study group was lower than that in the reference group,and the difference was statistically significant(P<0.05).Compared with the reference group,the whole blood viscosity,plasma viscosity and red blood cell volume of the study group were significantly lower,and the difference was statistically significant(t=4.460,5.445,8.117,P<0.001);there was no statistically significant difference between the two groups of adverse reactions(P>0.05).Conclusion The treatment of hyperacute cerebral infarction with hyperbaric oxygen+alteplase+edaravone is ideal,and it is worthy of promotion.
