戊酸雌二醇不同给药途径联合甲硝唑泡腾片对萎缩性阴道炎的疗效比较

Comparison of the Effects of Different Administration Routes of Estradiol Valerate Combined with Metronidazole Effervescent Tablets on Atrophic Vaginitis

目的比较戊酸雌二醇不同给药途径联合甲硝唑泡腾片对萎缩性阴道炎的疗效。方法选取2019年1月至2020年1月就诊于通许县中心医院的106例萎缩性阴道炎患者,按照随机数表法分为口服组、阴道组,各53例。两组均接受甲硝唑泡腾片治疗,口服组服用戊酸雌二醇治疗,阴道组戊酸雌二醇经阴道给药。比较两组疗效、阴道清洁度、阴道炎症因子水平、氧化应激指标及复发率。结果阴道组治疗总有效率高于口服组(96.23%比77.36%,P<0.05)。治疗3个月后,阴道组阴道清洁度优于口服组(P<0.05),阴道白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子α(TNF-α)水平低于口服组(P<0.05),血清丙二醛(MDA)、活性氧簇(ROS)水平低于口服组(P<0.05),超氧化物歧化酶(SOD)水平高于口服组(P<0.05)。两组复发率差异无统计学意义(P>0.05)。结论与口服给药相比,戊酸雌二醇经阴道给药联合甲硝唑泡腾片治疗萎缩性阴道炎疗效较好,能有效提高阴道清洁度,减轻阴道炎症及氧化应激。

Objective To compare the efficacy of different administration routes of estradiol valerate combined with metronidazole effervescent tablets on atrophic vaginitis.Methods A total of 106 patients with atrophic vaginitis who were admitted to Tongxu County Central Hospital from January 2019 to January 2020 were selected and divided into oral group and vaginal group with 53 cases in each group according to the random number table.Both groups were treated with metronidazole effervescent tablets,the oral group was treated with estradiol valerate,and the vaginal group was treated with estradiol valerate vaginally.The efficacy,vagina cleanliness,vaginal inflammatory factor levels,oxidative stress indicators and recurrence rate were compared between the two groups.Results The total effective rate of treatment in the vaginal group was higher than that in the oral group(96.23%vs.77.36%,P<0.05).After 3 months of treatment,the vaginal cleanliness of the vaginal group was better than that of the oral group(P<0.05).The levels of vaginal interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor alpha(TNF-α)in the vaginal group were lower than those in the oral group(P<0.05).Serum malondialdehyde(MDA)and reactive oxygen species(ROS)levels in the vaginal group were lower than those in the oral group(P<0.05).The level of superoxide dismutase(SOD)in the vaginal group was higher than that of the oral group(P<0.05).There was no statistical difference in the recurrence rate between the two groups(P>0.05).Conclusion Compared with oral administration,vaginal administration of estradiol valerate combined with metronidazole effervescent tablets has a better effect in the treatment of atrophic vaginitis,which could effectively improve vaginal cleanliness and reduce vaginal inflammation and oxidative stress.
