静脉补铁治疗在慢性心力衰竭合并铁缺乏患者中的应用

Application of intravenous iron therapy in patients with chronic heart failure complicated with iron deficiency

由于各种原因,特别是铁调素的影响,慢性心力衰竭患者常合并铁缺乏,伴或不伴贫血。机体缺铁造成能量代谢异常,会加重心力衰竭的临床表现,造成恶性循环。补铁治疗可以纠正心力衰竭患者体内的缺铁状态。口服补铁吸收及利用度较低,静脉补铁速度快。多项研究表明,对于射血分数减低的心力衰竭患者,静脉补铁治疗可以改善心力衰竭临床症状、增加运动耐力以及提升生活质量,还有可能降低心力衰竭住院率及死亡风险。本文对静脉补铁治疗的特点以及静脉补铁治疗在慢性心力衰竭合并铁缺乏患者中的应用及研究进展进行综述。

Due to various reasons,especially the influence of hepcidin,patients with chronic heart failure often had iron deficiency,with or without anemia.Iron deficiency bring in abnormal energy metabolism and heart failure deterioration,that resulted in a vicious cycle.Iron supplementation can correct iron deficiency in patients with heart failure.The absorption and utilization of oral iron was unsatisfactory,while the intravenous iron supplementation was fast.Several studies had shown that intravenous iron therapy in heart failure patients can relieve clinical symptoms of heart failure,increase exercise endurance and improve the life quality,and may also reduce the rate of hospitalization and death risk of heart failure.The clinical application and development of intravenous iron therapy in heart failure patients with iron deficiency was summarized in this paper.
