原发性高血压患者血压变异性与房颤关系研究 被引量:4

The relationship between atrial fibrillation and blood pressure variability in patients with essential hypertension

摘要 目的探讨原发性高血压患者血压变异性与心房颤动的关系。方法纳入2018年1月至2020年5月在我院住院治疗的原发性高血压患者145例,其中房颤组54例,非房颤组91例,采集临床基线资料,并进行动态血压监测与心脏彩超检查,记录相关参数。行Logistic回归曲线分析探讨血压变异性与心房颤动的相关性。结果房颤组24h加权收缩压标准差(Wsd-24h-SBP)、24h加权舒张压标准差(Wsd-24h-DBP)高于非房颤组,差异有统计学意义(均p<0.05)。Logistic回归分析显示年龄、尿酸、左房内径指数(LADi)、左室质量指数(LVMi)、Wsd-24h-SBP、Wsd-24h-DB是房颤的独立危险因素。结论24h加权收缩压标准差、24h加权舒张压标准差是心房颤动的独立危险因素。 Objective To explore the relationship between blood pressure variability and atrial fibrillation in patients with essential hypertension.Methods One hundred and forty-five patients with essential hypertension admitted to our hospital from January 2018 to May 2020 were divided into AF group(n=54)and AF-free group(n=91).The patients accepted ambulatory blood pressure monitoring and echocardiology examination,and their clinical baseline data,echocardiographic parameters and ambulatory blood pressure were recorded.Logistic regression curve analysis was used to explore the correlation between blood pressure variability and atrial fibrillation.Results It was shown that 24 h weighted standard deviation of systolic blood pressure(Wsd-24 h-SBP)and 24 h weighted standard deviation of diastolic blood pressure(Wsd-24 h-DBP)in the AF group were higher than those in the AF-free group(all p<0.05).Logistic regression analysis showed that age,uric acid,left atrial diameter index(LADi),left ventricular mass index(LVMi),Wsd-24 h-SBP,Wsd-24 h-DB were independent risk factors for atrial fibrillation.Conclusions 24 h weighted standard deviation of systolic blood pressure and 24 h weighted standard deviation of diastolic blood pressure are independent risk factors for atrial fibrillation.
