摘要 目的探究子宫内膜腺癌组织中多聚腺苷二磷酸核糖聚合酶-1(PARP-1)与Ras-MEK-ERK信号通路相关分子的表达及临床意义。方法选取2012年6月至2015年6月于徐州医科大学附属医院妇科行手术治疗且术后病理证实为子宫内膜腺癌的78例、子宫内膜非典型增生的20例及正常子宫内膜的30例患者的组织蜡块。采用免疫组化法检测PARP-1、pERK及P21Ras在各内膜组织中的表达情况;分析三者与子宫内膜腺癌患者临床病理参数的相关性。采用Cox比例风险回归模型分析三者的表达水平与患者预后的关系。结果三组子宫内膜组织PARP-1、P21Ras、pERK蛋白阳性率比较差异有统计学意义(P<0.01),均为子宫内膜腺癌组织最高。相关性分析显示,PARP-1、P21Ras与pERK在子宫内膜腺癌组织中的表达两两呈正相关关系(P<0.05)。在子宫内膜腺癌组织中,PARP-1阳性表达在临床分期、组织分化程度比较差异有统计学意义(P<0.05);pERK阳性表达在临床分期、组织分化程度、肌层浸润深度比较差异有统计学意义(P<0.05);P21Ras阳性表达在组织分化程度比较差异有统计学意义(P<0.01)。PARP-1、P21Ras、pERK蛋白阳性表达者5年累积生存率低于阴性表达者(P<0.05)。多因素分析结果显示,组织分化程度、肌层浸润深度、PARP-1和P21Ras为患者预后的独立影响因素(P<0.05)。结论子宫内膜腺癌组织中PARP-1可能通过参与Ras-MEK-ERK信号通路影响子宫内膜癌的发生、发展,PARP-1、P21Ras可能是子宫内膜腺癌的潜在预后指标。 Objective To investigate the expression and clinical significance of polyadenosine diphosphate ribose polymerase-1(PARP-1) and Ras-MEK-ERK signaling pathway related molecules in endometrial adenocarcinoma tissue.Methods From June 2012 to June 2015,78 cases of endometrial adenocarcinoma, 20 cases of endometrial atypical hyperplasia, and 30 cases of normal endometrium were selected for surgical treatment in the Department of Gynecology, Affiliated Hospital of Xuzhou Medical University.Immunohistochemical method was used to detect the expression of PARP-1,pERK and P21 Ras in the endometrial tissues of three groups;the correlation between PARP-1,pERK,P21 Ras and the clinicopathological parameters of patients with endometrial adenocarcinoma was analyzed.The COX proportional hazard regression model was used to analyze the relationship between the expression levels of the three and the prognosis of patients.Results The positive rates of PARP-1, P21 Ras, and pERK proteins in three groups of endometrial tissues were significantly different(P<0.01), all of which were the highest in endometrial adenocarcinoma tissues.Correlation analysis showed that the expressions of PARP-1,P21 Ras and pERK in endometrial adenocarcinoma tissue were positively correlated(P<0.05).In endometrial adenocarcinoma tissues, the positive expression of PARP-1 was significantly different in the clinical stage and the degree of tissue differentiation(P<0.05);the positive expression of pERK was different in the clinical stage, the degree of tissue differentiation, and the depth of myometrial invasion There was statistical significance(P< 0.05);the difference of P21 Ras positive expression in the degree of tissue differentiation was statistically significant(P<0.01).The 5-year cumulative survival rate of those with positive expression of PARP-1,P21 Ras and pERK protein was lower than that of those with negative expression(P<0.05).The results of multivariate analysis showed that the degree of tissue differentiation, the depth of muscle invasion, PARP-1 and P21 Ras were independent factors influencing the prognosis of patients(P<0.05).Conclusions PARP-1 in endometrial adenocarcinoma tissue may affect the occurrence and development of endometrial cancer by participating in the Ras-MEK-ERK signaling pathway.PARP-1 and P21 Ras may be potential prognostic indicators for endometrial adenocarcinoma.
