二甲双胍联合阿卡波糖对2型糖尿病患者糖脂代谢和胰岛素功能的影响

Effects of metformin combined with acarbose on glucose and lipid metabolism and insulin function in patients with type 2 diabetes

目的 观察二甲双胍联合阿卡波糖对2型糖尿病(T2DM)患者糖脂代谢和胰岛素功能的影响。方法 选择2018年7月-2019年10月于广西壮族自治区钟山县人民医院就诊的T2DM患者138例,采用随机数字表法分为观察组和对照组各69例。2组患者均给予阿卡波糖胶囊治疗,观察组患者加用盐酸二甲双胍缓释片治疗。比较2组患者治疗前后血糖[空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)]、血脂[总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]、空腹胰岛素(FINS)水平及胰岛素抵抗指数(HOMA-IR)。结果 治疗12周后,2组患者FBG、2 h PG、Hb A1c、TC、TG、LDL-C、FINS和HOMA-IR均低于治疗前,HDL-C水平均高于治疗前,且观察组降低/升高幅度大于对照组,差异均有统计学意义(P<0.01)。结论 二甲双胍联合阿卡波糖治疗T2DM可有效改善患者糖脂代谢和胰岛素功能,控制血糖水平。

Objective To observe the effects of metformin combined with acarbose on glucose and lipid metabolism and insulin function in patients with type 2 diabetes(T2DM).Methods 138 T2DM patients who were treated at Zhongshan County People's Hospital,Guangxi Zhuang Autonomous Region from July 2018 to October 2019 were selected,and they were randomly divided into observation group and control group,each of 69 cases.Both groups were treated with acarbose capsules,and the observation group was treated with metformin hydrochloride sustained-release tablets.The blood glucose[fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1c)],blood lipid[total cholesterol(TC),triacylglycerol(TG),Low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C)],fasting insulin(FINS)levels and insulin resistance index(HOMA IR)were compared between the two groups before and after treatment.Results After 12 weeks of treatment,the FBG,2 hPG,HbA1c,TC,TG,LDL-C,FINS and HOMA-IR in the two groups were lower than before treatment,and HDL-C levels were higher than before treatment,and the decrease/increasing amplitude of the observation group was greater than that of the control group,and the differences were statistically significant(P<0.01).Conclusion Metformin combined with acarbose can effectively improve the glucose and lipid metabolism and insulin function in patients with T2DM,and control the blood sugar level.
