艾灸治疗类风湿性关节炎的研究探析

Research and Analysis of Moxibustion in Treating Rheumatoid Arthritis

目的:探讨当前研究阶段类风湿性关节炎(Rheumatoid Arthritis,RA)治疗中应用艾灸疗法的可行性和科学性,以及下一阶段的研究方向。方法:计算机检索中国期刊全文数据库(CNKI)、维普数据库(VIP)、万方数据资源系统(Wanfang)、中国生物医学文献数据库(Sino Med),收集近十年关于艾灸治疗RA临床和机制研究的文献进行讨论分析。结果:从临床研究角度,艾灸疗法能起到缓解关节疼痛、肿胀、改善关节活动、缓解晨僵、降低相关血清学指标以及增强西药疗效的作用;从机制研究角度,艾灸疗法可通过对机体免疫细胞的影响,调节细胞因子的表达水平,调控各路信号通路等,改善早期类风湿性关节炎的病理变化。结论:从外周调节的角度来看艾灸疗法是一项高效且性价比高的非药物治疗,具有良好的研究基础。下一阶段我们应该探讨艾灸治疗早期RA的中枢神经机制,以及更加深入地揭示艾灸治疗RA在外周-中枢系统之间的联动作用机制,为艾灸治疗RA提供更精准的评价靶点,具有重要的科学意义和临床指导价值。

Objective:To explore the feasibility and scientificity of moxibustion therapy in rheumatoid arthritis(RA),and the research direction of the next stage.Methods:Computer retrieval of CNKI,VIP,Wanfang,China BioMedical Literature database(SinoMed)was conducted to collect and analyze the literatures on clinical and mechanism research of moxibustion therapy for RA in recent ten years.Results:From the perspective of clinical research,moxibustion can relieve joint pain,swelling,improve joint activity,relieve morning stiffness,reduce relevant serological indicators and enhance the efficacy of western medicine;from the perspective of mechanism research,moxibustion therapy can improve the pathological changes of early rheumatoid arthritis by influencing the immune cells of the body,regulating the expression level of cytokines,regulating various signal pathways.Conclusion:From the perspective of peripheral regulation,moxibustion therapy is a highly effective and cost-effective non-drug therapy,which has a good research foundation.In the next stage,we should explore the central nervous mechanism of moxibustion in the treatment of RA,and reveal the mechanism of the interaction between the peripheral and central systems of moxibustion in the treatment of early RA.It will provide more accurate evaluation targets for moxibustion in the treatment of RA,which has important scientific significance and clinical guidance value.
