血清转化生长因子β和骨膜蛋白与非瓣膜性心房颤动的关系

Relationship between serum transforming growth factor-β,periostin and non-valvular atrial fibrillation

目的分析血清转化生长因子β(TGF-β)、骨膜蛋白与非瓣膜性心房颤动(NVAF)的关系。方法选取2018年1月至2020年6月于安徽省芜湖市第二人民医院随诊的NVAF患者120例作为NVAF组,另选取同期门诊体检者56例作为对照组。比较2组对象血清TGF-β、骨膜蛋白和左心房内径及一般临床资料的差异,分析TGF-β、骨膜蛋白、左心房内径之间的相关性,应用Logistic回归方法分析NVAF的危险因素。结果 NVAF组患者血清TGF-β、骨膜蛋白、左心房内径及高血压病、糖尿病比例均明显高于对照组[(4.9±1.1)μg/L比(3.4±0.9)μg/L、(78±18) ng/L比(65±14) ng/L、(39±7) mm比(31±5) mm、60.8%(73/120)比39.3%(22/56)、40.8%(49/120)比25.0%(14/56)](均P <0.05)。Pearson相关性分析显示,NVAF组患者血清TGF-β与骨膜蛋白、左心房内径均呈明显正相关(r=0.732、0.701,均P <0.001),骨膜蛋白与左心房内径亦呈明显正相关(r=0.668,P <0.001)。Logistic多因素回归分析显示,高血压、TGF-β、左心房内径均是NVAF的危险因素(均P <0.001)。结论 NVAF患者血清TGF-β、骨膜蛋白水平升高,二者与左心房内径均呈正相关,且TGF-β是NVAF的独立危险因素。

Objective To analyze the relationship between serum transforming growth factor-β(TGF-β),periostin and non-valvular atrial fibrillation(NVAF).Methods From January 2018 to June 2020,120 patients with NVAF admitted to Wuhu Second People’s Hospital,Anhui Province were selected as NVAF group,and another 56 cases who underwent outpatient physical examination during the same period were selected as control group.The differences of serum TGF-β,periostin,left atrial diameter(LAD)and general clinical data between the two groups were compared.The correlation between TGF-β,periostin and LAD was analyzed.Logistic regression method was used to analyze the risk factors of NVAF.Results The serum TGF-β,periostin,LAD levels and the proportions of hypertension and diabetes in NVAF group were significantly higher than those in control group[(4.9±1.1)μg/L vs(3.4±0.9)μg/L,(78±18)ng/L vs(65±14)ng/L,(39±7)mm vs(31±5)mm,60.8%(73/120)vs 39.3%(22/56),40.8%(49/120)vs 25.0%(14/56)](all P<0.05).Pearson correlation analysis showed that serum TGF-βwas positively correlated with periostin and LAD(r=0.732,0.701,both P<0.001),and periostin was positively correlated with LAD(r=0.668,P<0.001).Multivariate Logistic regression analysis showed that hypertension,TGF-βand LAD were risk factors for NVAF(all P<0.001).Conclusions The levels of serum TGF-βand periostin in patients with NVAF are increase,which are positively correlated with LAD.TGF-βis an independent risk factor of NVAF.
