宣肺补益汤联合注射用还原型谷胱甘肽治疗慢性阻塞性肺疾病急性加重期痰湿阻肺兼肺肾气虚型的疗效及对肺功能和血气指标的影响

Efficacy of Xuanfei Buyi Decoction combined with glutathione injection on acute exacerbation of chronic obstructive pulmonary disease of phlegm-dampness obstructing lung and lung-kidney qi deficiency and its influence on lung function and blood gas indicator

目的观察宣肺补益汤联合注射用还原型谷胱甘肽治疗慢性阻塞性肺疾病急性加重期(AECOPD)痰湿阻肺兼肺肾气虚型的疗效及对肺功能和血气指标的影响。方法将150例AECOPD痰湿阻肺兼肺肾气虚型患者按照随机数字表法分为2组,对照组75例予西医常规治疗,治疗组75例在对照组治疗基础上应用宣肺补益汤联合注射用还原型谷胱甘肽治疗。2组均7 d为1个疗程,治疗2个疗程。比较2组疗效;比较2组治疗前后肺功能指标、血气指标、不良反应发生情况。结果治疗组总有效率94.67%(71/75)),对照组总有效率81.33%(61/75)),治疗组疗效优于对照组(P <0.05)。2组治疗后用力肺活量(FVC)、第1 s用力呼气容积(FEV1)、FEV1/FVC、最大呼气中段流量(MMF)、最大呼气流量(PEF)水平均较本组治疗前升高(P <0.05),且治疗组升高更明显(P <0.05)。2组治疗后动脉血氧分压[p(O2)]、动脉血氧饱和度(SaO2)均较本组治疗前升高(P <0.05),动脉血二氧化碳分压[p(CO2)]较本组治疗前降低(P <0.05),且治疗组治疗后p(O2)、SaO2均高于对照组(P <0.05),p(CO2)低于对照组(P <0.05)。结论在西医常规治疗基础上应用宣肺补益汤联合注射用还原型谷胱甘肽治疗AECOPD痰湿阻肺兼肺肾气虚型,可改善患者肺功能,有效调节血气状态,无明显不良反应。

Objective To observe the efficacy of Xuanfei Buyi Decoction combined with glutathione injection on acute exacerbation of chronic obstructive pulmonary disease(AECOPD) of phlegm-dampness obstructing lung and lung-kidney qi deficiency and its influence on lung function and blood gas indicator.Methods Totally 150 patients with AECOPD of phlegm-dampness obstructing lung and lung-kidney qi deficiency were randomly divided into two groups,75 cases in each group.Patients in control group were treated with conventional western medicine,patients in treatment group were additionally treated with Xuanfei Buyi Decoction combined with glutathione injection on the basis of control group.Both groups were treated for 2 courses,7 days as 1 course.Curative effect,pulmonary index,blood gas indicator and adverse reactions were compared between groups before and after treatment.Results The total effective rates in treatment group were better than those in control group [94.67%(71/75) vs 81.33%(61/75),(P < 0.05),respectively].Compared with before treatment,pulmonary index(FVC,FEV1,FEV1/FVC,MMF and PEF) in groups were higher after treatment(P < 0.05),which in treatment group were significantly increased(P < 0.05);arterial partial pressure of oxygen[p(O2) ]and blood oxygen saturation(SaO2) in groups were higher(P <0.05), which in treatment group were significantly increased(P < 0.05);while arterialpartialpressureof carbon dioxide[p(CO2) ]in groups was lower,which in treatment group were significantly decreased(P < 0.05).Conclusion For patients with AECOPD of phlegm-dampness obstructing lung and lung-kidney qi deficiency,Xuanfei Buyi Decoction combined with glutathione iniection on the basis of conventional western medicine can improve lung function,effectively regulate blood gas indicator and no obvious adverse reactions.
