血液透析、血液透析滤过与高通量血液透析治疗对糖尿病慢性肾病患者血糖、血压及肾功能的影响 被引量:10

EFFECTS OF HEMODIALYSIS,HEMODIAFILTRATION AND HIGH FLUX HEMODIALYSIS TREATMENT ON BLOOD SUGAR,BLOOD PRESSURE AND RENAL FUNCTION IN PATIENTS WITH DIABETIC CHRONIC KIDNEY DISEASE

摘要 目的比较分别应用血液透析(HD)、血液透析滤过(HDF)、高通量血液透析(HFHD)治疗糖尿病慢性肾病(DCKD)的价值。方法将90例DCKD患者按治疗方式不同分成HD组、HDF组与HFHD组各30例,各组采取对应方式透析治疗4周,比较透析前后的血糖、血压与肾功能指标水平变化。结果治疗后三组患者FPG、2hPG、24hSBP、24hDBP、BUN、SCR水平均较治疗前降低,治疗后HFHD组HDF组>HD组,两两组间比较差异有统计学意义(P<0.05)。结论应用HFHD治疗DCKD可取得满意的降糖、降压效果,同时显著改善患者肾功能。 Objective To compare the value of hemodialysis(HD),hemodiafiltration(HDF)and high-throughput hemodialysis(HFHD)in the treatment of diabetic chronic kidney disease(DCKD).Methods Ninety patients with DCKD were divided into HD group,HDF group and HFHD group(30 cases in each group)according to different treatment methods.Each group was treated with dialysis in the corresponding way for 4 weeks,and the changes of blood glucose,blood pressure and renal function indexes before and afterdialysis were compared.Results After treatment,the levels of FPG,2 hPG,24 hSBP,24 hDBP,BUN and Scr in the three groups were all decreased compared with before treatment.After treatment,the levels of HFHD group were lower than that of HDF group<HD group,and the difference between the two groups was statistically significant(P<0.05).After treatment,GFR levels in all three groups were lower than those before treatment.After treatment,>in the HFHD group and>HD in the HDF group showed statistically significant differences between the two groups(P<0.05).Conclusion The application of HFHD in the treatment of DCKD can achieve satisfactory effect of hypoglycemia and blood pressure,and significantly improve the renal function.
