摘要
目的分析俯卧位机械通气在儿童重症肺炎治疗中的效果及应用价值。方法在2017年1月至2019年2月于本院就诊的儿童重症肺炎患儿中选择符合此次研究入组标准以及排除标准的70例患儿作为研究对象,依据随机数字表法完成分组后实施治疗,对照组35例接受常规仰卧位机械通气治疗,观察组35例采取俯卧位机械通气治疗。比较两组的治疗效果。结果观察组临床治疗总有效率比对照组更高,差异有统计学意义(P<0.05)。治疗后,观察组患儿的PaO2水平和PaCO2均显著优于对照组,差异有统计学意义(P<0.05)。观察组患儿的临床症状及体征消失时间显著短于对照组,差异有统计学意义(P<0.05)。结论使用俯卧位机械通气为小儿重症肺炎患儿提供治疗可达到更理想效果,有利于改善患儿的动脉血气指标,更好的增加氧供,利于气道分泌物引流,且有利于促进临床症状及体征的消失,促进患儿康复,实现尽早撤机。
Objective To analyze the effect and application value of prone position mechanical ventilation in the treatment of severe pneumonia in children.Methods From January 2017 to February 2019,70 children with severe pneumonia who met the criteria for inclusion and exclusion in this study were selected as the research subjects.The treatment was carried out after completion of grouping according to the digital table method.The control group(35 cases)received conventional supine mechanical ventilation while the observation group(35 cases)received prone position mechanical ventilation.The effect of the two groups was compared.Results The total effective rate of clinical treatment in the observation group was higher than that in the control group,and the difference was significant(P<0.05).After treatment,the PaO2 level and PaCO2 in the observation group were significantly better than those in the control group,and the difference was significant(P<0.05).Comparing the disappearance time of clinical symptoms and signs between the two groups,the results also showed that the observation group was significantly shorter than the control group,and the comparison difference was statistically significant(P<0.05).Conclusion The use of prone position mechanical ventilation to provide clinical treatment for children with severe pneumonia can achieve a more ideal therapeutic effect,is conducive to improving the arterial blood gas index of children,can better increase oxygen supply,is conducive to the drainage of airway secretions,and is conducive to promoting the disappearance of clinical symptoms and signs,promoting the recovery of children,is conducive to the early withdrawal of the machine.
