分级诊疗背景下淮安市某社区居民基层首诊意愿调查

Investigation on Willingness of First-visit at Grassroots Medical Institution Among Residents in a Community of Huai'an City Under the Background of Hierarchical Medical Care

目的:了解淮安市某社区居民基层首诊意愿及其影响因素,为分级诊疗的推行提供现实依据。方法:采用现场调查和网络调查相结合的方法对淮安市某社区居民进行问卷调查,用χ^(2)检验和二分类logistic回归分析社区居民基层首诊意愿的影响因素。结果:愿意进行基层首诊的社区居民占比为63.8%;社区居民对基层首诊的知晓度和认可度分别为24.3%和31.0%;回归分析结果显示,年龄、职业、医保类型、基层医疗卫生机构满意度是影响社区居民基层首诊意愿的主要因素(P<0.05)。结论:社区居民基层首诊意愿比较高,但对基层首诊的知晓度和认可度均比较低,参加医保和对基层医疗卫生机构的高满意度能够提高社区居民的基层首诊意愿。

Objective:To understand the status and influencing factors of willingness of first-visit at grassroots medical institution among residents in a community of Huai'an city,so as to provide a practical basis for the implementation of hierarchical medical care.Methods:A questionnaire survey was conducted among residents in a community of Huai'an city by the method of combining field investigation and network investigation.Chi-square test and binary logistic regression were used to analyze the influencing factors of community residents'willingness of first-visit at grassroots medical institution.Results:63.8%of the community residents were willing to seek first-visit at grassroots medical institution.The awareness rate and recognition degree of community residents to seek first-visit at grassroots medical institution were 24.3%and 31.0%respectively.The results of regression analysis showed that age,occupation,medical insurance type and satisfaction degree of primary health organizations are the main influencing factors of community residents'willingness to seek first-visit at grassroots medical institution(P<0.05).Conclusion:The community residents'willingness to seek first-visit at grassroots medical institution is relatively high,but the awareness rate and recognition rate of first-visit at grassroots medical institution are relatively low.Participation in medical insurance and high level of satisfaction with grassroots medical institution can increase the community residents'willingness to seek first-visit at grassroots medical institution.
