老年非小细胞肺癌患者外周血TLR2、TLR4水平与肿瘤增殖、侵袭基因表达的相关性

摘要 目的探究老年非小细胞肺癌(NSCLC)患者外周血Toll样受体(TLR)2、TLR4水平与肿瘤增殖、侵袭基因表达的相关性。方法取2018年2月至2020年12月于我院收治的123例老年NSCLC患者,按照病理检查结果及TNM肿瘤分期分为早期(Ⅰ期)组27例、中期(Ⅱ~Ⅲ期)组66例、晚期(Ⅳ期)组30例,同时选取于我院体检中心进行体检的健康者50例作为对照组。检测三组老年NSCLC患者肿瘤组织及癌旁组织中增殖基因、侵袭基因mRNA表达量,并测定所有研究对象的外周血单核细胞中TLR2、TLR4 mRNA表达;采用Spearman相关性分析明确外周血单核细胞中TLR2、TLR4 mRNA表达与老年NSCLC肿瘤恶性生物学行为的相关关系。结果晚期组TLR2、TLR4、PAK6、Bin1、CD59、PAX6、HOXB7 mRNA水平均明显高于早期组、中期组以及对照组,且中期组水平均明显高于早期组、对照组,早期组水平亦均明显高于对照组,两两比较组间差异具有统计学意义(P<0.05)。晚期组caspase-8、LRRC3B mRNA水平均明显低于早期组、中期组以及对照组,且中期组水平均明显低于早期组、对照组,早期组水平亦均明显低于对照组,两两比较组间差异具有统计学意义(P<0.05)。经Spearman相关性分析发现,外周血TLR2、TLR4 mRNA水平与老年NSCLC患者肿瘤组织增殖基因CD59、PAX6、HOXB7 mRNA水平呈正相关,与LRRC3B mRNA表达量呈负相关(P<0.001);与肿瘤组织侵袭基因PAK6、caspase-8 mRNA表达量呈正相关,与Bin1 mRNA表达量呈负相关(P<0.001)。结论外周血单核细胞中TLR2、TLR4 mRNA水平越高,老年NSCLC分期及肿瘤恶性程度越高,有助于对老年NSCLC患者诊断、治疗和预后判断,值得推广应用。 Objective To explore the correlation between Toll like receptor(TLR) 2 and TLR4 levels in peripheral blood and the expression of tumor proliferation and invasion genes in elderly patients with non-small cell lung cancer(NSCLC).Methods 123 elderly patients with NSCLC admitted to our hospital from February, 2018 to December, 2020 were divided into the early stage(stage Ⅰ) group(n=27), middle stage(stage Ⅱ-Ⅲ) group(n=66) and late stage(stage IV) group(n=30) according to the pathological examination results and TNM tumor staging. In addition, 50 healthy people were selected as the control group. On the day of admission, the mRNA expressions of proliferation genes and invasion genes in tumor tissues and adjacent tissues of three groups of elderly patients with NSCLC were detected, and the mRNA expressions of TLR2 and TLR4 in peripheral blood mononuclear cells of all subjects were determined. Spearman correlation analysis was applied to determine the correlation between the mRNA expressions of TLR2 and TLR4 in peripheral blood mononuclear cells and malignant biological behavior of elderly patients with NSCLC.Results The mRNA levels of TLR2, TLR4, PAK6, Bin1, CD59, Pax6, and HOXB7 in the late stage group were significantly higher than those of the early group, the middle group and the control group, and the middle-term group were significantly higher than those of the early group and the control group, and the early group were also significantly higher than those of the control group, and the difference for each pair was statistically significant(P<0.05). Caspase-8 and LRRC3 B mRNA levels in the late group were significantly lower than those of the early group, the medium group and the control group, and the middle group were significantly lower than those of the early group and the control group, and the early group were also significantly lower than those of the control group, and the difference for each pair was statistically significant(P<0.05). Spearman correlation analysis showed that TLR2 and TLR4 mRNA levels in peripheral blood were positively correlated with CD59, Pax6 and HOXB7 mRNA levels in tumor tissues of elderly patients with NSCLC, while negatively correlated with LRRC3 B mRNA expression(P<0.05). They were also positively correlated with PAK6 and caspase-8 mRNA expression in tumor tissues, while negatively correlated with Bin1 mRNA expression(P<0.05).Conclusion The higher the mRNA levels of TLR2 and TLR4 in peripheral blood mononuclear cells, the higher the stage and malignant degree of elderly patients with NSCLC, which can be helpful to the diagnosis, treatment and prognosis of elderly patients with NSCLC.
