摘要
目的探究消化性溃疡(PU)并发上消化道出血(UGIB)患者CD4^(+)CD25^(+)T细胞、白细胞介素-17(IL-17)、转化生长因子-β(TGF-β)水平变化及意义。方法选择2019年4月至2021年4月我院诊治的112例PU患者,根据是否并发UGIB分为溃疡组62例和溃疡出血组60例,另选择同期健康体检的50名志愿者为健康组,均行血常规检查,采用流式细胞仪检测CD4^(+)CD25^(+)T细胞水平,采用酶联免疫吸附法检测IL-17、TGF-β水平,分析指标变化和意义。结果血红蛋白(Hb)、红细胞(RBC)水平比较溃疡出血组<溃疡组<健康组(P<0.05),CD4^(+)CD25^(+)、IL-17、TGF-β水平比较溃疡出血组>溃疡组>健康组(P<0.05),两两比较差异均有统计学意义(P<0.05);治疗后,溃疡出血组患者Hb、RBC水平显著升高(P<0.05),CD4^(+)CD25^(+)、IL-17、TGF-β水平显著降低(P<0.05);PU并发UGIB患者CD4^(+)CD25^(+)、IL-17、TGF-β水平均与Hb(r=-0.883、-0.802、-0.843,P<0.05)、RBC(r=-0.769、-0.716、-0.764,P<0.05)水平呈负相关。结论PU患者并发UGIB后,患者CD4^(+)CD25^(+)、IL-17、TGF-β水平均异常升高并均与Hb、RBC水平呈负相关,三者水平越高患者贫血风险越大。
Objective To explore changes and significance of levels of CD4^(+)CD25^(+)T cell,interleukin-17(IL-17)and transforming growth factor-β(TGF-β)in patients with peptic ulcer(PU)complicated with upper gastrointestinal bleeding(UGIB).Methods 112 patients with PU who were diagnosed and treated in the hospital were selected between April,2019 and April,2021,and they were divided into the ulcer group(62 cases)and ulcer with bleeding group(60 cases)according to whether they were complicated with UGIB.In addition,50 volunteers who underwent physical examination during the same period were selected as the healthy group.Routine blood examination was performed for all subjects,and the level of CD4^(+)CD25^(+)T cell was detected by flow cytometry,and levels of IL-17 and TGF-βwere measured by enzyme-linked immunosorbent assay.Changes and significance of these indicators were then analyzed.Results Comparisons of levels of hemoglobin(Hb)and red blood cell(RBC)were shown as following:ulcer with bleeding group<ulcer group<healthy group(P<0.05),and comparisons of levels of CD4^(+)CD25^(+),IL-17 and TGF-βdisplayed as:ulcer with bleeding group>ulcer group>healthy group(P<0.05),and the differences between any two groups were statistically significant(P<0.05).After treatment,levels of Hb and RBC in the ulcer with bleeding group were significantly increased(P<0.05),while levels of CD4^(+)CD25^(+),IL-17 and TGF-βwere significantly decreased(P<0.05).Levels of CD4^(+)CD25^(+),IL-17 and TGF-βwere negatively correlated with Hb(r=-0.883,-0.802,-0.843,P<0.05)and RBC(r=-0.769,-0.716,-0.764,P<0.05)in patients with PU complicated with UGIB.Conclusion When patients with PU are complicated with UGIB,levels of CD4^(+)CD25^(+),IL-17 and TGF-βare abnormally increased and are negatively correlated with levels of Hb and RBC.The higher the levels of these three markers,the greater the risk of anemia for patients.
