Optimization of Delay Time of Time Density Curve Method in 64-slice CT Head and Neck CTA Examination

摘要 目的:探讨64排CT头颈血管造影(CTA)检查中时间密度曲线法延时时间的优化。方法:回顾性分析2018年6月-2020年4月182例头颈CTA检查患者的临床资料,根据时间密度曲线延迟时间将其分为A组(n=91)和B组(n=91)。A组检查中时间密度曲线法延时时间为显像达峰时间+4 s,B组检查过程中时间曲线法延时时间为显像达峰时间+对比剂团注时间-扫描时间。采用ADW4.5工作站完成图像后处理,重建出头颈部动脉血管的三维立体影像,绘制时间密度曲线,并借助曲线进一步优化延长时间。结果:两组颈总动脉平均CT值比较差异无统计意义(P>0.05);B组颈内动脉、大脑中动脉平均CT值高于A组,差异有统计学意义(P<0.05);时间曲线显示,随着延时时间的增加,血管密度HU开始上升,并在10~18 s时达到峰值,随后随整体呈下降趋势并呈波动样变化,并趋于稳定;A组扫描时时间密度曲线法延时时间为显像达峰时间+4 s,获得的图像产生比较多的图像伪影,难以通过剪切秀出,影响对疾病的诊断;B组扫描获得的图像相对清晰,伪影亦相对较少,部分图像可以通过剪切等方式完全去除伪影,不影响疾病诊断。结论:64排CT头颈CTA检查中时间曲线法延时时间为显像达峰时间+对比剂团注时间-扫描时间,能更加容易获得最佳扫描时相,有助于提高图像质量。 Objective:To explore the optimization of delay time of time density curve method in 64-slice CT head and neck angiography(CTA)examination.Method:The clinical data of 182 patients who underwent head and neck CTA examinations from June 2018 to April 2020 were retrospectively analyzed.They were divided into group A(n=91)and group B(n=91)according to the delay time of time density curve.The delay time of time density curve method in group A was the imaging peak time+4 s,and the delay time of time density curve method in group B was the imaging peak time+the contrast agent bolus injection time-the scanning time.ADW4.5 workstation was used to complete image post-processing,and three-dimensional images of head and neck arteries were reconstructed,and the time density curve was drawn,and the curve was further optimized to extend the time.Result:There was no statistically significant difference in the average CT value of the common carotid artery between the two groups(P>0.05).The average CT value of the internal carotid artery and middle cerebral artery in group B were higher than those in group A,and the differences were statistically significant(P<0.05).The time curve showed that with the increase of the delay time,the vascular density HU began to increase,and reached the peak value at 10 to18 s,and then decreased with the overall trend and fluctuated,and tended to be stable.The delay time of time density curve method in group A scanning was the imaging peak time+4 s,and the obtained images produces more image artifacts,which was difficult to show through cutting,affected the diagnosis of disease.The images obtained by group B scanning were relatively clear,and the artifacts were relatively few.Some images can be completely removed by means of cutting,without affected the diagnosis of disease.Conclusion:In the 64-slice CT head and neck CTA examination,the delay time of time curve method is the imaging peak time+the contrast agent bolus injection time-the scanning time,which can more easily obtain the best scanning phase and help improve the image quality.
