身痛逐瘀汤内服联合伤科黄水外敷对内固定治疗胫骨平台骨折患者膝关节功能及IL-6、CRP水平的影响

Effect of Internal Administration of Shentong Zhuyu Decoction Combined with Shangke Huangshui External Application on Knee Joint Function and IL-6,CRP Levels in Patients with Tibial Plateau Fracture Treated by Internal Fixation

目的:探讨身痛逐瘀汤内服联合伤科黄水外敷对内固定治疗胫骨平台骨折患者膝关节功能及IL-6、CRP水平的影响。方法:选取2016年4月-2020年4月在笔者所在医院接受内固定治疗的68例胫骨平台骨折患者作为研究对象,根据随机数字表法将其分为观察组(n=34)与对照组(n=34)。对照组接受伤科黄水外敷治疗,观察组在对照组基础上使用身痛逐瘀汤内服治疗。比较两组膝关节功能、炎性因子水平及血清生化指标。结果:治疗后,两组Rasmussen评分均较治疗前明显升高(P<0.05),且观察组明显高于对照组,差异有统计学意义(P<0.05)。治疗后,两组白介素-6(IL-6)、C反应蛋白(CRP)水平均较治疗前明显降低(P<0.05),且观察组IL-6、CRP水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清神经生长因子(NGF)及碱性磷酸酶(ALP)水平均较治疗前明显升高(P<0.05),且观察组NGF、ALP水平均明显高于对照组,差异有统计学意义(P<0.05)。结论:将身痛逐瘀汤内服联合伤科黄水外敷应用于胫骨平台骨折患者内固定治疗中可有效改善患者膝关节功能,降低IL-6、CRP水平,效果较好。

Objective:To investigate the effects of internal administration of Shentong Zhuyu Decoction combined with external application of Shangke Huangshui on knee joint function,IL-6 and CRP levels in patients with tibial plateau fracture treated by internal fixation.Method:A total of 68 patients with tibial plateau fracture who received internal fixation treatment in our hospital from April 2016 to April 2020 were selected as the research objects,and were divided into the observation group(n=34)and the control group(n=34)according to the random number table method.The control group was treated with external application of Shangke Huangshui,and the observation group was treated with internal administration of Shentong Zhuyu Decoction on the basis of the control group.The knee joint function and inflammatory factor level and serum biochemical index were compared between the two groups.Result:After treatment,the Rasmussen scores of the two groups were significantly increased compared with those before treatment(P<0.05),and the observation group was significantly higher than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6)and C-reactive protein(CRP)in the two groups were significantly decreased compared with those before treatment(P<0.05),and the levels of IL-6 and CRP in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of serum nerve growth factor(NGF)and alkaline phosphatase(ALP)in the two groups were significantly increased compared with those before treatment(P<0.05),and the levels of NGF and ALP in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Internal administration of Shentong Zhuyu Decoction combined with external application of Shangke Huangshui can effectively improve the knee joint function of patients with tibial plateau fracture treated by internal fixation,and reduce IL-6 and CRP levels,with good effect.
