5种呼吸道病原体IgM抗体检测对呼吸道感染的诊断价值 被引量:1

摘要 目的探讨呼吸道病原体谱IgM抗体检测对呼吸道感染的诊断价值。方法回顾性分析2018年2月—2020年2月苏州市中西医结合医院收治的1 431例呼吸道感染患者的病历资料,用间接免疫荧光法检测肺炎支原体、肺炎衣原体、呼吸道合胞病毒、腺病毒、柯萨奇病毒B组共5种病原体的IgM。结果 1 431例呼吸道感染者中有490例检出呼吸道病原体IgM,总阳性率为34.24%。其中肺炎支原体IgM阳性率最高,为17.82%。IgM阳性样本中单一感染占69.39%,混合感染占30.61%。不同年龄段患者的肺炎支原体和腺病毒IgM阳性率差异有统计学意义(P<0.05),18岁以下患者阳性率高于其他年龄组(P<0.05);不同季节肺炎支原体和腺病毒IgM阳性率差异有统计学意义(P<0.05)。结论引起呼吸道感染的病原体以肺炎支原体为主,检测5种呼吸道病原体IgM能区分不同病原体感染,为临床诊治提供依据。 Objective To investigate the diagnostic value of IgM detection of respiratory pathogens in respiratory tract infection patients. Methods The medical information of 1 431 patients with respiratory tract infection treated in Suzhou Integrated Traditional Chinese and Western Medicine Hospital from February 2018 to February 2020 were retrospectively analyzed. IgM antibody against 5 kinds of respiratory pathogen were detected by using indirect immunofluorescence assay,including Mycoplasma pneumoniae,Chlamydia pneumoniae,respiratory syncytial virus,adenovirus and Cox B virus group. Results A total of 490 patients showed IgM positive among 1 431 patients with respiratory tract infection,the total positive rate was 34.24%. The positive rate of Mycoplasma pneumoniae IgM was the highest,17.82%. Among the 490 positive samples,69.39% was single infection,and 30.61% was mixed infection. The positive rate of Mycoplasma pneumonia and adenovirus IgM in different age groups was statistically significant(P<0.05),and the positive rate of less than 18-year-old group was higher than that in other groups. The positive rate of Mycoplasma pneumoniae and adenovirus IgM in different seasons was statistically significant(P<0.05). Conclusion Mycoplasma pneumoniae was the main pathogen causing respiratory tract infection. The detection of 5 kinds of respiratory pathogen IgM can distinguish different pathogen infection,and provide basis for clinical diagnosis and treatment.
