肝脏瞬时弹性成像技术在慢性乙肝抗病毒治疗中的应用价值评价

Evaluation of the practical value of liver transient elastography in the antiviral treatment of chronic hepatitis B

目的评价肝脏瞬时弹性成像技术在慢性乙型病毒性肝炎(慢性乙肝)抗病毒治疗中的应用价值。方法138例慢性乙肝患者作为研究对象,对患者实行抗病毒治疗的过程中采用肝脏瞬时弹性成像技术进行检测。比较治疗前后肝硬度值及临床指标(谷丙转氨酶、透明质酸、层粘连蛋白、乙肝病毒脱氧核糖核酸、血清)变化情况。结果治疗后,患者肝硬度值(7.64±1.23)kPa明显小于治疗前的(13.77±2.65)kPa,差异有统计学意义(P<0.05)。治疗后,患者谷丙转氨酶(23.39±4.36)U/L、透明质酸(66.43±6.68)μg/L、层粘连蛋白(95.28±5.24)μg/L、乙肝病毒脱氧核糖核酸(90.52±4.87)copies/ml、血清Ⅲ型前胶原(91.35±5.17)ng/ml明显低于治疗前的(147.46±11.47)U/L、(107.17±9.55)μg/L、(142.47±11.26)μg/L、(125.75±8.59)copies/ml、(140.77±10.88)ng/ml,差异有统计学意义(P<0.05)。结论在慢性乙肝患者的抗病毒治疗中肝脏瞬时弹性成像技术能够对患者的肝纤维化程度进行有效判断,同时有着重复性高、无创及操作便捷等优势,可以将其作为检测肝硬化及肝纤维化的重要方式。

Objective To evaluate the practical value of liver transient elastography in the antiviral treatment of chronic hepatitis B.Methods There were 138 patients with chronic hepatitis B as study subjects,and the liver transient elastography technology was used to detect during the antiviral treatment.The changes in liver stiffness values and clinical indicators(alanine aminotransferase,hyaluronic acid,laminin,hepatitis B virus deoxyribonucleic acid,serum typeⅢprocollagen)before and after treatment were compared.Results After treatment,the liver stiffness value(7.64±1.23)kPa of patients was significantly lower than(13.77±2.65)kPa before treatment,and the difference was statistically significant(P<0.05).After treatment,the alanine aminotransferase(23.39±4.36)U/L,hyaluronic acid(66.43±6.68)μg/L,laminin(95.28±5.24)μg/L,hepatitis B virus deoxyribonucleic acid(90.52±4.87)copies/ml,serum typeⅢprocollagen(91.35±5.17)ng/ml were significantly lower than(147.46±11.47)U/L,(107.17±9.55)μg/L,(142.47±11.26)μg/L,(125.75±8.59)copies/ml,(140.77±10.88)ng/ml before treatment,and the difference was statistically significant(P<0.05).Conclusion In the antiviral treatment of chronic hepatitis B patients,liver transient elastography can effectively determine the degree of liver fibrosis in patients,and has advantages such as high repeatability,non-invasive and convenient operation,which can be used as an important method for the detection of cirrhosis and liver fibrosis.
