超声乳化联合小梁切除术治疗老年青光眼合并白内障的临床疗效

Clinical efficacy of phacoemulsification combined with trabeculectomy in the treatment of senile glaucoma complicated with cataract

目的探讨老年青光眼合并白内障患者应用超声乳化联合小梁切除术治疗的临床疗效。方法80例老年青光眼合并白内障患者,随机分为联合组和常规组,每组40例。常规组患者采用小梁切除术治疗,联合组患者采用超声乳化联合小梁切除术治疗。比较两组患者临床疗效,治疗后眼压和视力。结果常规组患者的总有效率为67.50%(27/40),联合组患者的总有效率为92.50%(37/40);联合组患者治疗总有效率明显高于常规组,差异具有统计学意义(P<0.05)。联合组患者治疗后眼压为(13.6±3.4)mm Hg(1 mm Hg=0.133 kPa),低于常规组的(15.4±3.7)mm Hg,差异具有统计学意义(t=2.266,P<0.05)。联合组患者治疗后视力为(0.8±0.1),高于常规组的(0.5±0.1),差异具有统计学意义(t=13.416,P<0.05)。结论老年青光眼合并白内障患者采用超声乳化联合小梁切除术治疗临床疗效显著,患者的视力与眼压得到显著提升,具有临床推广应用价值。

Objective To discuss the clinical efficacy of phacoemulsification combined with trabeculectomy in the treatment of senile glaucoma complicated with cataract.Methods A total of 80 senile patients with glaucoma and cataract were randomly divided into combination group and conventional group,with 40 cases in each group.The conventional group was treated with trabeculectomy,and the combination group was treated with phacoemulsification and trabeculectomy.The clinical efficacy,and intraocular pressure and visual acuity after treatment were compared between the two groups.Results The total effective rate of the conventional group was 67.50%(27/40),which was 92.50%(37/40)of the combination group.The total effective rate of the combination group was obviously higher than that of the conventional group,and the difference was statistically significant(P<0.05).After treatment,the intraocular pressure of the combination group was(13.6±3.4)mm Hg(1 mm Hg=0.133 kPa),which was lower than(15.4±3.7)mm Hg of the conventional group,and the difference was statistically significant(t=2.266,P<0.05).After treatment,the visual acuity of the combination group was(0.8±0.1),which was higher than(0.5±0.1)of the conventional group,and the difference was statistically significant(t=13.416,P<0.05).Conclusion Combination of phacoemulsification and trabeculectomy has significant clinical effect in the treatment of senile glaucoma complicated with cataract,and the visual acuity and intraocular pressure of the patients have been significantly improved,which has clinical promotion and application value.
