血清hs-CRP水平在学龄期严重脓毒症患儿连续性血液净化治疗临床预后中的意义

Significance of Serum hs-CRP Level in Clinical Prognosis of Continuous Blood Purification Treatment in School-age Children with Severe Sepsis

目的:探讨血清超敏C-反应蛋白(hs-CRP)水平与学龄期严重脓毒症患儿连续性血液净化治疗临床预后的关系。方法:前瞻性选取2019年7月~2020年10月在某院治疗的102例学龄期严重脓毒症患儿作为研究对象,患儿均行连续性血液净化治疗;检测治疗前患儿血清hs-CRP水平,分析血清hs-CRP水平与学龄期严重脓毒症患儿连续性血液净化治疗效果的相关性。结果:102例严重脓毒症患儿治疗1周后显效率为45.10%(46/102),有效率为38.23%(39/102),无效率为16.67%(17/102);预后不良组血清hs-CRP水平较预后良好组升高,差异有统计学意义(P<0.05);Logistics回归分析结果显示,hs-CRP水平高表达可能与学龄期严重脓毒症连续性血液净化治疗预后不良相关(OR>1,P<0.05);ROC曲线结果显示,hs-CRP水平预测学龄期严重脓毒症患儿连续性血液净化治疗预后的AUC>0.7。结论:血清hs-CRP水平与学龄期严重脓毒症患儿连续性血液净化治疗临床预后密切相关,可为临床预后评估提供依据。

Objective:To investigate the relationship between the serum high-sensitivity C-reactive protein(hs-CRP)level and the clinical prognosis of continuous blood purification treatment in school-age children with severe sepsis.Methods:102 school-age children with severe sepsis who were treated in a hospital from July 2019 to October 2020 were prospectively selected as the research objects.All the children underwent continuous blood purification treatment,and the serum hs-CRP level of the children before treatment was detected;The correlation between serum hs-CRP level and the effect of continuous blood purification treatment in school-age children with severe sepsis was analyzed.Results:The efficiency of 102 children with severe sepsis after treatment was 45.10%(46/102),the effective rate was 38.24%(39/102),and the inefficiency was 16.67%(17/102)after 1 week of treatment;The serum hs-CRP level in the poor prognosis group was better than that in the good prognosis group,and the difference was statistically significant(P<0.05).The logistic regression analysis showed that the high expression of hs-CRP may be related to the poor prognosis of school-age severe sepsis continuous blood purification treatment(OR>1,P<0.05).The ROC curve results showed that hs-CRP levels predicted AUC>0.7 of prognosis of continuous blood purification treatment in school-age children with severe sepsis.Conclusion:The serum hs-CRP level is closely related to the clinical prognosis of continuous blood purification treatment in school-age children with severe sepsis,and can provide a basis for clinical prognosis evaluation.
