胶原蛋白敷料与常规敷料治疗糖尿病足溃疡的比较

Comparison of Collagen Dressing and Conventional Dressing in the Treatmentof Diabetic Foot Ulcer

目的:将胶原蛋白敷料应用于糖尿病足溃疡治疗中,探讨其临床治疗效果和安全性。方法:选择2019年1月~2020年12月某院住院治疗的糖尿病足溃疡94例,随机分为对照组(常规敷料治疗)和观察组(胶原蛋白敷料治疗)各47例,比较两组治疗前和治疗第1周、2周、4周、8周足部溃疡面积、愈合率,统计愈合时间和不良事件发生率。结果:治疗前、治疗第1周,对照组、观察组足部溃疡面积比较无差异(P>0.05);治疗第2周、治疗第4周、治疗第8周,观察组足部溃疡面积低于对照组(P<0.05)。治疗第4周、第8周,观察组创面愈合率显著高于对照组(P<0.05),观察组总愈合时间及Wagner不同分级患者愈合时间均低于对照组(P<0.05);对照组、观察组均发生灼烧感、发热、皮疹、创面红肿等不良事件,且两组不良事件率比较,差异无统计学意义(P>0.05)。结论:与常规敷料治疗相比,胶原蛋白敷料对糖尿病足溃疡创面面积的缩小有显著作用,可加速创面愈合,提高了不同Wagner分级患者的创面愈合时间,不良反应事件少。

Objective:To investigate the clinical efficacy and safety of collagen dressing in the treatment of diabetic foot ulcer.Methods:94 cases of diabetic foot ulcer in a hospital from January 2019 to December 2020 were randomly divided into control group(conventional dressing treatment,n=47)and observation group(collagen dressing treatment,n=47).The ulcer area,healing rate,healing time and incidence of adverse events were compared between the two groups before treatment and the first,second,fourth and eighth week of treatment.Results:Before treatment and the first week of treatment,there was no significant difference in the foot ulcer area between the control group and the observation group(P>0.05).At the second week,the fourth week and the eighth week of treatment,the foot ulcer areas of the observation group were lower than those of the control group(P<0.05).At the fourth week and the eighth week of treatment,there was statistical difference in wound healing rate between the two groups(P<0.05).The total healing time and the healing time of patients with different Wagner levels on the observation group were lower than those in the control group(P<0.05).The control group and the observation group had burning sensation,fever,rash,wound swelling and other adverse events,and there was no statistically significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion:Compared with conventional dressing,collagen dressing can significantly reduce the wound area of diabetic foot ulcer,accelerate the wound healing,improve the wound healing time of patients with different Wagner grades,and reduce the adverse reactions.
