静脉丙种球蛋白联合蒲地蓝口服液在5岁以下重症手足口病患儿中的应用及对炎症因子的影响研究

Application of Intravenous Gamma Globulin Combined with Pudilan Oral Liquid in Children under 5 years Old with Severe Hand-foot-mouth Disease and Its Influence on Inflammatory Factors

目的:探讨静脉丙种球蛋白联合蒲地蓝口服液在5岁以下重症手足口病患儿中的应用效果及对炎症因子的影响。方法:选择2014年1月~2018年12月5岁以下重症手足口病患儿120例,采用抽签法分为对照组和观察组各60例,对照组采用静脉丙种球蛋白治疗,观察组在对照组基础上联合蒲地蓝口服液治疗,两组均完成15d治疗,比较两组症状消失时间、炎症因子及药物安全性。结果:观察组治疗后退热、疱疹消退、神经症状消失、住院及溃疡消失时间均短于对照组(P<0.05);两组治疗后15d炎症因子低于治疗前(P<0.05);观察组药物联合使用15d后炎症因子IL-1β、IL-6、NSE、hs-cTn及S100B蛋白水平低于对照组(P<0.05);两组药物治疗过程中不良反应发生率无统计意义(P>0.05)。结论:静脉丙种球蛋白联合蒲地蓝口服液在5岁以下重症手足口病患儿中能缩短其病情好转时间,可降低炎症联级反应,安全性较高,值得推广应用。

Objective:To investigate the effect of intravenous gamma globulin combined with Podilan oral liquid in children under 5 years of age with severe hand-foot-mouth disease and its influence on inflammatory factors.Methods:From January 2014 to December 2018,120 children with severe hand-foot-mouth disease under 5 years of age were selected as subjects,and they were divided into two groups by lottery,with 60 cases in each group.The control group was treated with intravenous gamma globulin,and the observation group was treated with podilan oral liquid on the basis of the control group.Both groups completed 15 days of treatment.The time to symptom disappearance,inflammatory factors and drug safety were compared between the two groups.Results:The time of fever,herpes regression,loss of neurological symptoms,hospitalization and loss of ulcers after treatment in the observation group was shorter than that in the control group(P<0.05).15 days after treatment,the inflammatory factors in the two groups were lower than before treatment(P<0.05).After 15 days of combined drug use,the levels of inflammatory factors such as IL-1β,IL-6,NSE,hs-cTn and S100B protein in the observation group were lower than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Intravenous gamma globulin combined with Podilan oral liquid can shorten the time of symptom improvement and reduce the level of inflammatory factors in children with severe hand-foot-mouth disease under 5 years old,with high safety,which is worthy of popularization and application.
