LA联合NT-proBNP、LVEDD联合NT-proBNP对慢性心力衰竭患者的诊断价值

The diagnostic value of NT-proBNP,LA,LVEDD,LA combined with NT-proBNP,LVEDD combined with NT-proBNP in patients with chronic heart failure

探究慢性心力衰竭患者氨基末端脑钠肽前体(NT-proBNP)与心功能指标的相关性,分析NT-proBNP、左房内径(LA)、左室舒张末期内径(LVEDD)、LA联合NT-proBNP、LVEDD联合NT-proBNP对慢性心力衰竭患者的诊断价值。方法:回顾性分析2016年6月~2019年6月石河子大学医学院第一附属医院心血管内科收治654例诊断为慢性心力衰竭的住院患者作为试验组,收集同时期健康体检患者50例作为对照组。记录患者一般资料,实验室检查、心功能指标,采用受试者工作特征(ROC)曲线分析LA联合NT-proBNP、LVEDD联合NT-proBNP对慢性心力衰竭患者的诊断价值。结果:慢性心力衰竭组患者与健康对照组患者在性别、收缩压、BMI、三酰甘油、LDL-C方面比较,差异均无统计学意义(P>0.05);在年龄、舒张压、心率、LA、RV、LVEDD、LVEF、NT-proBNP、HB、NLR、肌酐、总胆固醇、HDL-C方面比较,差异均有统计学意义(P<0.05)。NT-proBNP与LA、LVEDD呈正相关。LA联合NT-proBNP、LVEDD联合NT-proBNP诊断慢性心力衰竭的ROC曲线下面积(0.968、0.937)大于NT-proBNP(0.934)、LA(0.881)、LVEDD(0.811)。结论:NT-proBNP与LA、LVEDD具有相关性,LA联合NT-proBNP、LVEDD联合NT-proBNP对慢性心力衰竭患者的诊断价值较单一指标临床价值更大,LA联合NT-proBNP、LVEDD联合NT-proBNP对慢性心力衰竭患者的诊断可作为早期辅助参考指标。

Objective To explore the correlation between NT-proBNP and cardiac function indicators in patients with chronic heart failure,and analyze the diagnostic value of NT-proBNP,LA,LVEDD,LA combined with NT-proBNP,LVEDD combined with NT-proBNP in patients with chronic heart failure.Method A retrospective analysis was performed on 654 inpatients diagnosed with chronic heart failure admitted to the Cardiology department of the First Affiliated Hospital of Shihezi University Medical College from June 2016 to June 2019,they served as the experimental group,and 50 healthy physical examination patients in the same period as the control group.The general data of the patients,laboratory examination and cardiac function indicators were recorded,and the ROC curve was used to analyze the diagnostic value of LA combined with NT-proBNP and LVEDD combined with NT-probNP in patients with chronic heart failure.Results There were no statistically significant differences in gender,systolic blood pressure,BMI,triglyceride and LDL-C between patients in chronic heart failure group and healthy control group(P>0.05);The differences in age,diastolic blood pressure,heart rate,LA,RV,LVEDD,LVEF,NT-proBNP,HB,NLR,creatinine,total cholesterol,HDL-C were statistically significant(P<0.05).NT-proBNP was positively correlated with LA and LVEDD.The area under ROC curve(0.968,0.937)of LA combined with NT-proBNP and LVEDD combined with NT-proBNP to diagnose chronic heart failure was greater than that of NT-proBNP(0.934),LA(0.881)and LVEDD(0.811).Conclusion There is a correlation between NT-proBNP and LA and LVEDD.LA combined with NT-proBNP and LVEDD combined with NT-proBNP has a greater clinical value than a single indicator in the diagnosis of patients with chronic heart failure.LA combined with NT-proBNP and LVEDD combined with NT-proBNP can be used as an early auxiliary reference indicator for the diagnosis of patients with chronic heart failure.
