摘要
目的分析营养风险筛查-2002(NRS-2002)和微型营养评价精法(MNA-SF)对老年肺结核患者营养状况的预测价值。方法选取我院499例老年结核病患者作为研究对象,入院后第1天采用NRS-2002和MNA-SF评估所有患者的营养状况。采用Pearson相关分析检验两种评价方法与传统营养相关指标之间的相关性。结果NRS-2002、MNA-SF分别筛出有营养风险的患者127例(25.45%)、164例(32.86%)。单因素分析结果显示,在NRS-2002、MNA-SF评估下,两组的BMI、ALB、TP水平、CD3^(+)、TSF、菌检结果比较,差异具有统计学意义(P<0.05)。Logistic回归分析结果显示,BMI、ALB、TP、CD3^(+)、TSF、菌检结果均是营养风险评估结果的独立影响因素(P<0.05)。NRS-2002、MNA-SF与传统营养相关指标呈显著相关(P<0.001)。NRS-2002与MNA-SF评估营养风险结果的一致性极好,Kappa值=0.764。结论NRS-2002、MNA-SF均是评估老年肺结核患者营养状况的有效工具,两者一致性较好。老年肺结核患者营养风险检出率高,对于这类具有营养风险的患者应积极进行营养干预。
Objective To analyze the predictive value of nutritional risk screening-2002(NRS-2002)and short-form mini-nutritional assessment(MNA-SF)on nutritional status of elderly patients with pulmonary tuberculosis.Methods A total of 499 elderly patients with tuberculosis in our hospital were selected as the research objects,and the nutritional status of all patients was evaluated by NRS-2002 and MNA-SF on the first day after admission.Pearson correlation analysis was used to test the correlation between the two evaluation methods and traditional nutrition-related indicators.Results NRS-2002 and MNA-SF screened 127(25.45%)and 164(32.86%)patients with nutritional risk respectively.Univariate analysis result showed that under the evaluation of NRS-2002 and MNA-SF,there were significant differences in BMI,ALB,TP,CD3^(+),TSF and bacterial test between the two groups(P<0.05).Logistic regression analysis result showed that BMI,ALB,TP levels,CD3+,TSF and bacterial test results were independent factors influencing the results of nutritional risk assessment(P<0.05).NRS-2002,MNA-SF and traditional nutrition-related indicators significantly correlated(P<0.001).The results of nutritional risk assessment by NRS-2002 and MNA-SF were in excellent consistency,Kappa value=0.764.Conclusion Both NRS-2002 and MNA-SF are effective tools for assessing the nutritional status of elderly pulmonary tuberculosis patients,and the two have good consistency.The detection rate of nutritional risk in elderly pulmonary tuberculosis patients is high,and nutritional intervention should be actively carried out for these patients with nutritional risk.
