微创肺叶切除术和肺段切除术治疗非小细胞肺癌的安全性及对患者免疫功能、肺功能的影响

Safety of minimally invasive lobectomy and segmentectomy in the treatment of non-small cell lung cancer and their influences on immune function and pulmonary function of patients

目的分析微创肺叶切除术和肺段切除术治疗非小细胞肺癌的安全性及对患者免疫功能、肺功能的影响。方法选取我院收治的100例非小细胞肺癌患者为研究对象,根据手术方式不同将其分为肺叶组(48例,微创肺叶切除术)和肺段组(52例,微创肺段切除术)。比较两组的免疫功能指标、肺功能指标、并发症发生情况。结果术前,两组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、NK比较,差异无统计学意义(P>0.05);术后,肺段组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、NK高于肺叶组,差异具有统计学意义(P<0.05)。术前,两组的FEV1%、FVC%、MVV%比较,差异无统计学意义(P>0.05);术后,两组的FEV1%、FVC%、MVV%均较术前降低,但肺段组高于肺叶组,差异具有统计学意义(P<0.05)。两组的并发症总发生率比较,差异无统计学意义(P>0.05)。结论微创肺叶切除术和肺段切除术治疗非小细胞肺癌均具有一定的安全性,但肺段切除术对患者免疫功能、肺功能的改善作用更佳。

Objective To analyze the safety of minimally invasive lobectomy and segmentectomy in the treatment of non-small cell lung cancer and their influences on immune function and pulmonary function of patients.Methods One hundred patients with non-small cell lung cancer admitted in our hospital were selected as the research objects.The patients were divided into lung lobe group(48 cases,minimally invasive lobectomy)and lung segment group(52 cases,minimally invasive segmentectomy)by different operation mode.The immune function indexes,pulmonary function indexes and complications were compared between the two groups.Results Before operation,there were no significant differences in CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and NK between the two groups(P>0.05);after operation,the CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and NK in the lung segment group were higher than those in the lung lobe group,and the differences were statistically significant(P<0.05).Before operation,there were no significant differences in FEV1%,FVC%and MVV%between the two groups(P>0.05);after operation,FEV1%,FVC%and MVV%in the two groups were lower than those before operation,but those in the lung segment group were higher than the lung lobe group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Both minimally invasive lobectomy and segmentectomy in the treatment of non-small cell lung cancer are safe.But segmentectomy has a better improvement effect on the immune function and pulmonary function of patients.
